There are plenty of apps that explain exercises or allow you to log your lifts, but can anyone recommend one that lays out a block or undulating periodization routine? I see a few on the App Store but they don't have many reviews. I feel like I've hit a plateau and want to start mixing things up.

small notebook and paper will be the best one... or you can just use a notepad app and manually write it down

mixing things up doesn't really make sense.

What are your goals? Bodybuilding or powerlifting?

You can:
Eat more
Drop sets
Strip sets
Add sets
Reverse your workout
Increase/decrease rest times
Increase/decrease days/time at the gym
Deload
 
Specifically, I'm wondering if there's an app that lays out block or linear periodization. I already record my lifts, and I vary my rep range, but since I work out alone and don't want to pay a trainer, I'd like to have a program to follow -- you know, micro, macro, and messocycles, etc., that will force me to train differently and hopefully increase my gains.

Check out the Mountain Dog Diet app. It's a great app, and the guy that responds to questions on it is named Marc Snyder, great dude and gets in contact with you pretty fast. App has 2 years worth of programs loaded into it and gives you the option to build and program extra days and has a ton and I mean a ton of work outs on there. Also has an option to track all your macros and will give you suggested meals. It's 29 bucks a month but it's a great option to use and honestly man I love it, the work outs are brutal at times... They give you a free 1 week trial to see if you like it.
