I have pretty much had it with general practitioner doctors. Im leaving out specialists as i have had no issues with them yet.
But at my last appt with my GP last week, i went in for my regular checkup. I asked him about trying out a different medication for my adhd. I asked about trying modafinil(a schedule III drug) to see if i liked it better than the 30mg adderal(a schedule II drug) i was on. He said thats for narcolepsy. i said yes i understand that, but its also for sleep apnea as well which i have. He didnt really like that to much.
He said im the doctor here. And i replied "well whats with all the commercials saying, ask your doctor about this medication and that medication". He went silent for a moment then said it was irrelavent.
Then he just snapped and said you want me to be your doctor anymore? "you do this everytime you come in here".
I replied by saying "i do what, ask questions because im conserned about my health"
He then proceeded to write out the last of my scripts and that was that.
I do admit that i enjoy studying many different aspects of health. Maybe i bruied his ego with a question he couldnt answer or that i knew stuff he didnt expect me to know?
Just curious if anyone else has had this problem??? or was it my fault for not keeping my mouth shut so the doctor could get in and out as fast as possible??

as soon as i hear that " I'm the doctor here" line it's time to find a new doctor.

Modafinil does nothing to treat the underlying cause for the sleepiness in sleep apnea and I would have not prescribed either and told you to drink a cup of coffee instead to improve wakefulness.

I'm not familiar with the exact script. you're referring to...However it can never hurt to get a second, third, or even a fourth opinion. I got lucky so far with my primary care Dr. Although I don't ask for much, he is always understanding and takes care of any issue I have. *knock on wood*

I apologize for the doctor bro. When you practice the same way over and over for a few years you stop looking at alternatives and think "my way or the highway." Please don't lose faith in the medical systems. There are amazing doctors out there that ARE dedicated to their professions have no ego and will take the time to teach you, LEARN FROM YOU and research. I am not saying they will always agree with you but they will be open to new suggestions.

Having said that do realize these doctors have YEARS of training. EvEN a GP goes through 4 years of medical school plus 3 years of residency and thats not including his bachelors. PLUS however many years they have been practicin. While they might not be open to new suggestions they have seen a lot of stuff and USUALLY are on target. EMPHAASIS on usually.

as soon as i hear that " I'm the doctor here" line it's time to find a new doctor.

Exactly my same thoughts, too many good Dr's out there to pass up. :)

I've worked in the Pharma business for 20 plus years and in many different therapeutic categories. I'm an informed patient to say the least. It never scores points with my m. Deities.

And Im also respectful. Everyone in medicine s stressed. The docs have the government and managed cate telling them how to prescribe. They hate direct to consumer advertising, and they get annoyed when patients ask for drugs by name. IMHO and experience. not all docs of course.

Provigil is also an expensive branded medication. Not a bad choice for ADHD, although i dont believe its indicated for adhd or sleep apnea. Generic adderral is cheap. Managed care likes cheap. So they put pressure on docs to write adderral and not provigil. Unless you are paying cash for your meds. Then the doc should not care.

with all do respect, if you look up modafinil it clearly states its use for sleep apnea as well as narcolepsy. Drink a cup pf coffee?? just curious if you personally have sleep apnea and know about the seriousness of it?? I forgot to mention that the doctor also felt the need to write me scripts for prozac, wellbutrin, clonopin, and as a result of the side effects of the prozac i needed levitra he said.

I can understand your side of things. the problem is that i never had an attitude or acted like i knew more than the doctor. i just asked questions which i thought your supposed to do when you see your doctor!

I dont call them m. Deities as a compliment. Many are real jerks. Heres a question that doesn't need answering, (very personal)does the doc know you are on gear? What other controlled prescriptions are you requesting? That never scores points with a doc.

I agree with your post. i think we have the best healthcare of any country. especially when it comes to emergency situations. Its just dissapointing that in my experience with doctors, there resolution to everything is a medication.

I grew up a skinny average size kid, and as far back as i can remember i always had problems with being tired. I could sleep all day long. In fact, all through school i slept in every one of my classes(passed thanks to copying homework). but i went to doctors with this problem and everytime it was my fault because i needed to go to bed earlier or get on a schedule when the doctor had no idea about my sleeping times etc.
And then they would write me scripts for ambien and even tried antidepressants.
well at the age of 27 years old i got fed up with it. i did my own research. Plus my wife also gave me input about how i was sleeping.
So i demanded a sleep apnea test. Doctor said i dont have it cause im not overweight.
first night in the sleep lab...... 6 hours- 260 apneas(apnea means waking up due to not breathing because of airway blockage but not concious of it).
No wonder i have so many issues. but doctors dont take it serious because there is a treatment for sleep apnea that doesnt involve drugs.
And the truth is that 90% of most GP doctors patients qualify for a sleep study. trust me, i have done my research. it will eventually kill you if untreated.

Yes I have had sleep apnea for 7+ years have posted on its negative effects several times on the board.

What are you doing to address your sleep apnea? The underlying cause of it.

Modafinil is prescribed to treat the tiredness caused by sleep apnea and nothing to do with actually helping and improving sleep apnea. Treat the cause and you wont be as tired to require a drug to keep you awake.;)

I actually started going to that doctor to get off pain meds prescribed by another doctor, by using suboxine. the funny thing though is that this doctor who kicked me out has a reputation in town of prescribing very high amounts of oxy's

Ive had it treated for 5 years now with cpap. the question is what kind of damage was done to say my brain for being oxygen deprived for so many years?? maybe i have adhd because of it. or is adhd a made up disease to sell drugs? just food for thought. im not trying to argue with you buddy. im on the same side. i wish there was more awareness for sleep apnea. Dont you??

ABSTRACT

Patients with obstructive sleep apnea/hypopnea syndrome can experience residual daytime sleepiness despite regular use of nasal continuous positive airway pressure therapy. This randomized, double-blind, placebo-controlled, parallel group study assessed the efficacy and safety of modafinil for the treatment of residual daytime sleepiness in such patients. Patients received modafinil (n = 77) (200 mg/d, Week 1; 400 mg/d, Weeks 2 to 4) or matching placebo (n = 80) once daily for 4 wk. Modafinil significantly improved daytime sleepiness, with significantly greater mean changes from baseline in Epworth Sleepiness Scale scores at Weeks 1 and 4 (p < 0.001) and in multiple sleep latency times (MSLT) at Week 4 (p < 0.05). The percentage of patients with normalized daytime sleepiness (Epworth score < 10) was significantly higher with modafinil (51%) than with placebo (27%) (p < 0.01), but not for MSLT (> 10 min; 29% versus 25%). Headache (modafinil, 23%; placebo, 11%; p = 0.044) and nervousness (modafinil, 12%; placebo, 3%; p = 0.024) were the most common adverse events. During modafinil or placebo treatment, the mean duration of nCPAP use was 6.2 h/night, with no significant change from baseline observed between groups. Modafinil may be a useful adjunct treatment for the management of residual daytime sleepiness in patients with obstructive sleep apnea/hypopnea syndrome who are regular users of nasal continuous positive airway pressure therapy.

Having read your comment again i thought about the fact that doctors have to do so many hours of continued education. maybe someone can fill me in on what these CE's usually consist of? are they mostly just learning about new drugs? or???

OK.... that information looks just like what i researched. And again i want to stress that i just asked the doctor about the medication and if it was worth trying out to see if it worked better than 30mg adderall. His reply was "no, its for narcolepsy and you have add". I even read where modafinil helped with depression as does adderall. but i guess he rather me be on prozac which i hate and tried to come off of and got very sick. even at the lowest dose.
