So I went to see the cardiologist to check out the small pericardial infusion that showed on a CT I had done last month. He did ekg and so far everything sounds good he is doing an eco tomorrow just to check everything.

My question is what things can he find in an eco. I figure size of heart and all that. But if there is any blood clots or blockage of any kind will eco show? I've never had an heart eco done

It shouldn't show clots or blockage. Just chamber sizes and how efficient it beats. So you'll find out your systolic function (ejection fraction) and diastolic function and things like that.

Really won't show blockage or clot in a coronary artery. The best they can do is perform a muga scan using a shot of epinephrine or a stress echo. They need to get heart rate way up, if there's a lack of blood flow in the coronaries then problems would show up. Many times the heart can look normal otherwise.

A good analogy is that an EKG is like looking at the electrical of a house, while the echocardiogram is like looking at the plumbing.
 
I asked for a eco, for my own intel and knowledge (perhaps opening Pandora's box here)

Anyway, for what it's worth, he mentioned it wasn't enough so he has me doing a eco, EKG test, stress test along with a 48 hour heart monitor test.

Is the muga scan done like a CT? I've had a few CT done that they inject something into the IV with a machine. This liquid gives you a hot flush that last a few seconds.

I think in some cases, they can see a clot or plaque, but it isn't as reliable. You're lucky they found it in yours honestly.

A TTE is more reliable for blood clots.

Yea when I asked the cardiologist if he can see them with eco he said yes and the girl that did the eco on me. Said yes too. I don't see why they would lie so I believe them. But I can see what you guys are saying. There are other test that are better at that

I thought that the only way they could see a blockage in a coronary would be to do an angiogram or indirectly view it with a nuclear scan. With the nuclear scan they inject you with a radioactive tracer and then view your heart in an imaging machine. I think it might be a CT but not sure. Basically the areas of your heart that aren't receiving enough oxygen, hypoxic, are a different color. I think those are black if they aren't getting much at all while parts that get good flow are red/yellow etc. I also think they inject you with epinepherine to get your heart rate up, simulates exercise.

Ive had them done but it has been many years. I also had a PET scan done too once.

A CT Angiogram (non-invasive) and/or coronary angiogram (invasive) could be used to definitively determine if CAD is extant. An echo might detect things like a blood clot on a heart valve, as could an LV Angiogram, which goes into the lv chamber with a pig-tailed catheter. In this scenario, a large volume of contrast dye is first injected into the ventricle. However, this catheter could also injure the aortic valve, as it passes through in a coil.

These additional tests should be a last resort, because of the radiation etc (as mentioned). If there are no heart valve murmurs, your blood pressure, ejection fraction, heart wall thickness, diameter and heart wall motion are normal, it doesn't suggest blocked arteries.
