Everytime I take an AI and E2 goes down a lil too much, i pretty much get direhea arter I eat meals. I can feel it, it's like it's not being digested well and I go to bathroom and poop it out. Get a mild stomach pain like aye it's time to let out

What could be the cause?

My wife is on tamoxifen from having breast cancer, she has zero E2 and does not have that problem. So I doubt it.

Tamox is a serm not AI

With tamoxifen there is still E2 in the body. Just blocks the receptor. Not saying his gastro issues are from the AI however
 
Sounds what like they now call IBS (irritable bowel syndrome). I would research that on the net. I don't know much of anything about it.

Some kittens I once bought had IBS and it turned out to be some kind of bacterial in their body. We had to try about 5 different antibiotics before we killed it all. They are normal now. Its a long shot, but you could have some kind of microbe in you that is causing it. Have you traveled to any foreign countries since they symptoms started?

Gastrointestinal issues are one of the little talked about side effects of AIs. Arimidex is the most common cause and diarrhea is the most common effect. Many people just get soft or loose stools from adex but never notice it. It is more common if you have been using it for a long time, it's like an allergy. I used adex on and off over a decade before it started giving me issues, now I can only use aromasin, arimidex gives me the shits bad.

It's a known side effect from AIs, you a can try switching to another one that might not cause it (letro) or just use nolva instead (not recommended, long-term tamoxifen use is probably not good).

Agreed, I would like to see more conversation about gastro issues, gut microbiome and how it is affected.

For example, people say that injectable versions of oral steroids avoid stomach distress (nausea, acid reflux and appetite issues)... but no one likes to say that it might be due to avoiding contact with the gut microbiota. and intestinal lining.
People don't understand we have more bacteria in us than actual human cells. We really are a symbiotic multi-organism.

I think in the coming years as more research comes out, people will hopefully consider their intestinal flora in the same light as liver care or heart health.
