Free T and Total T question

What is the better of the two?

Someone with 1,200 total T with a 30 free test level. Compared to

Someone with 350 total T and 120 free test.

I know free test levels are what we are looking for so it baffles me how someone can have a low total levwl with such greater free test. This are real results I've read on forums.

The "Free Hormone Hypothesis" is just that - a hypothesis. So we can't say for sure the relative impact of free vs. total testosterone on a given individual. We also need to take individual differences into account.

That being said, if it were me, I would prefer Option 1.

Total T includes free plus protein-bound testosterone concentration. The normal range in most laboratories is 300 to 800 ng/dL. Total T should be the first test ordered in the workup for hypogonadism. Free T is most useful when there is a reason to suspect that the sex hormone- binding globulin level is abnormally decreased, leading to potentially falsely low total T readings. This may occur particularly in older or obese patients. When ordering serum free T, the lab doing the test should use equilibrium dialysis, because the more commonly used analog method often gives misleading information. Hope this helps.

The opposite has been my issue (pre trt), lowish total t, very low free t and high sbgh. So only looking at total t i have at times been in range, albeit very low. However, the very low total t is what drove my current urologist to prescribe treatment

total test is important but FREE test is key. free test is what does the work, many old guys have high total test but super low free test. thats why proviron works, to free up the test and it makes them feel better
