With you being polycythemic this would cause more destruction of red blood cells than normal, which would cause your bilirubin to be elevated. This would be the more likely cause. Dumping a pint of blood may correct this. You might have some gallstones if this isn't related to polycythemia. If there was one good thing of a slightly elevated bilirubin, you have a higher antioxidant/antiinflammatory activities. Don't use that as a plus tho. You need to evaluate the aforementioned with your PCP.
You definitely need to dump a pint of blood. I would suggest that you pursue a sleep study, as sleep apnea is suspected.
Your eGFR is of some concerns. Yet I'm more prone to pursue a Cystatin C assay over a eGFR+Creatinine. This is a more accurate assessment of Renal function. Unlike the calculation of eGFR+Creatinine it's not reflective of protien intake, hydration status or muscle mass. You may wanna speak to your physician about pulling this assay.
Your lipids, these need some addressing too.
