I am concerned over a few results....

I am running trt dosage of test prop, 100 mgs a week, 3 ius GH daily, 25 mg proviron (m,w,f) and 10 mg Cialis daily

My eGFR is at all time low (57), have been taking 4k grams of astragalus for only 3 weeks prior to test. I wanted to see if i could raise this number from previous test results. Past results have fluctuated between 70 and 63

Also my Prolactin is in the gutter. I am not taking caber, prami, etc... I am unsure how that occurs?

Also serum estrone (E1) is high, BUT other other estrogen results in range. Seems odd to me.

AST and ALT slightly outta range, been taking 1200 mg NAC daily, due to liver enzymes consistently slightly elevated.

Also TIBC and UIBC results are high. This is first time i have these checked.


Any help to get markers back in line is appreciated.

I take the following OTC supps.

astragalus, berberine, tumeric, nac, saw palmetto, and a multi vitamin, and CoQ10

TIBC and UIBC can be high from low iron. If you give blood too often could be related.
 
Make sure you are using a reputable brand of astragalus,I just did bloodwork using Nusapure astragalus, 4 caps AM/4 caps PM for 6 weeks and my gfr went from 70 to 69(yes,it actually worsened) I switched over to Vitamin Shoppe brand and will retest in another month or two.

isnt egfr calculated from creatinine and some body measurements like weight and hight? if this is the case then it is pretty much different every hour (dehydration etc)+
or am i wrong?

From what I understand it is calculated like you stated ,creatinine ,height ,weight and if you are black or white . When testing make sure you are hydrated each time to get an accurate reading .I don’t think it changes hourly but being dehydrated at the time of testing would skew the number

Your eGFR is plummeting, my advice to you is to stop all AAS and test your kidneys function in a month or 2.

If you stay on the juice, you're on your way to kidney failure.

He is on TRT right now and his test levels are in range. Crashing his test levels to below normal will be of no benefit to his kidney function.

Only thing there that would concern me is super low prolactin, and I'm not sure why that's happening, as well as the high estrone.

You egfr is a calculation which doesn't really take into account muscle mass, leanness, etc. we all run high Creatinine levels. but make sure and get good and hydrated before next test. My Cr went from 1.6 on a dehydrated test, to 1.2 on super hydrated test. I would not freak out about that at all.

You liver function tests look normal for someone who works out.

I was using NOW Brand. Ya , will be trying a different brand for sure.

Ya the prolactin number has me puzzled as well. I found an article on pub-med regarding low prolactin being correlated to low sex drive. I found this interesting since I literally have damn near no desire. It’s actually starting to frustrate my wife quite a bit, me as well.

I’m interested to hear when I go back to my Dr sometime in the next 2 weeks, what she will suggest.

- Estrone: I have no idea. I don't think this would lead to any adverse effects though.

- Prolactin: Similarly puzzling. It may be that the combination of high free T, borderline low E2, and Proviron results in too little estrogen signalling. I'd recommend dropping the Proviron altogether to balance the T/E ratio somewhat. Too high T/E ratio may also be the cause of libido problems.

- TIBC: Slight iron deficiency. Do you have GERD or any digestive issues and are you taking meds for either of those? If not, a high quality iron supplement will do the trick.

- eGFR: Nothing to panic about, but the trend downwards should be reason enough to follow up with more testing. How has your body weight changed going from the 70 eGFR to the current bloodwork, do you have more or less lean mass now? Also, how is your blood pressure now and how has it been in the past?

Regarding the recommendation to drink more before the test. You are only fooling yourself, getting a better number due to overhydration does not mean your kidney function is suddenly improved. You are simply masking things to make yourself feel better...
The reference range for Creatinine is based on normal (i.e. average person) hydration levels, which isn't much. So drink the same way you normally would before the test to get an accurate reading.

Re: eGFR, I have much more lean mass now. My BP is finally under control, in the past, even on LOSARTN (100 mg), was still high 140/90. I stopped taking the meds, and just been using CIALIS, my BP is now 120's / 70's. Will continue to monitor with more testing in future.

Re: Prolactin, I have found the cause, a month before the test, i stopped taking the caber i was using while running NPP. To be brutally honest, i had to go back in my logs and look to see if i was taking anything, my short term memory sucks. I am so pissed at myself for not catching this sooner. Sorry. So... based on these facts, i am assuming i used to much which obviously led to crashed Prolactin. I knew it had a long half life of 60 - 70 hours. But am surprised to see a month later discontinuing use how low my levels are. I will be stopping the Proviron usage.

Thanks for mentioning it, it completely changes the diagnosis. The average time until prolactin returns to pre-treatment levels after Caber use is 3 months (https://www.ncbi.nlm.nih.gov/pubmed/19336508), so it's likely that your low prolactin is due to the previous Caber use. The only remaining symptom to be explained is the low libido.
Based on the new information, I suspect the Saw Palmetto rather than a high T/E ratio is the culprit for the sexual dysfunction. The extract contains potent 5alpha-reductase inhibitors, meaning that it can cause sexual dysfunction similar to Finasteride. The combination of reduced 5alpha reductase activity and low levels of pregnenolone (due to HPTA suppression) can lead to markedly reduced production of neurosteroids, which in turn reduces GABA-A receptor activation, resulting in sexual dysfunction. This is on top of the negative effects of low DHT on sexual function.

Unless you suffer from BPH or androgen-dependent hair loss, I would drop the Saw Palmetto and keep the Proviron in. In fact, you could try increasing the Proviron dosing frequency in the short term (I know, complete reversal of previous recommendation). In addition, I would look into OTC pregnenolone supplementation, which is something that a couple TRT docs recommend anyways.

In the end, you should follow the instructions of your doctor, of course. Just throwing out some suggestions.
 
How high is your protein intake?

How long have you been at the trt dose?

When and what was your last cycle and how long were u on?
