Do my thyroid labs look good?

I've been working with my trt doc to try to optimize thyroid function.
The only previous lab I have was my TSH of 3.1mIU/L
I'm on TRT testosterone TRT and dropped the 6iu rHGH six weeks ago and added 60mg NP thyroid. Thyroid panel is attached. Most of the thyroid optimization stuff I've looked into say you want to aim for a TSH under one. Doc is fine with pushing my dose up if I want to try but I guess with free t3 getting up out of the reference range already maybe I shouldn't bump to 90mg or 120mg NP thyroid. I don't want to burn muscle mass or lose any strength doing this. What do you think?

Personally I wouldn't want my free T3 to increase much higher than that for thyroid replacement. Just to be clear you don't have any prior labs besides the TSH? Did they run the panels for thyroid antibodies this time? It could be useful to determine if it's autoimmune in some way.

Don't get too hung up on getting the TSH under 1 when you're at 1.5. It's more like a signaling hormone. The other numbers are much more important. For a rough example - if your LH output was on the low side, but your testosterone was 900, getting your LH higher wouldn't really matter since the LH is already having the desired effect.

Even though it's in range the rt3 does seem a little higher than ideal. When taking NP instead of taking T4/T3 separately you lose the ability to manipulate those variables individually a bit. A lower amount of T4 converting to Rt3 while keeping the Free t3 nice and high would probably have you feeling a bit better. But if you're feeling good I wouldn't change anything.

Also no, I doubt you're in any danger of losing muscle at this level especially being on trt.

Thanks for that. Really appreciate it. And yes about only having TSH before hand. When I originally went to my urologist it was for having a hard timing conceiving. I'm about to hit 40. Doc found I had low test and low sperm count. He got me fixed up and while reviewing my labs I brought from my PCP, which for thyroid was only TSH, he said hey if you aren't feeling better on TRT there is always room to improve your thyroid and see if that helps with the tiredness and brain fog. It's been two years since then so I decided it's time to tinker with the thyroid and see if I'm missing out on anything. So far it hasn't made a huge difference but then again neither did trt. But I've noticed small things improving slowly since I started the NP thyroid like less tired some days, more spontaneous erections, not even sure how that's related lol, and sleeping a little better/deeper. So I can't complain. I think after reading your response I'll just stay where I'm at and not try and mess with a good thing!

What kind of doctor Is prescribing this? Sounds like he Is just along for the ride and giving what you request.?

He's pretty cool. Got me on deca too bc I have some chronic knee pain. He was a urologist when I met him but now he just does hormone optimization

When did you take your medication last before the labs? Exogenous t3 will act very quickly and spike your labwork

Here is ft3, total t3, and TSH after a single 50mcg dose of liothyronine sodium for example

Here’s the study if you’d like to read more

Do you recall the rough mcg conversion of your NDT?

I took it at 7am and tested at 10:30am. I've read that NDT peaks slower and lasts longer than synthetic t3 which is why many people can get away with a single dose per day on NDT but synthetic t3 tends to do better on split or triple dosing.

The prescribing information says that if you are currently on 100mcg synthetic t4 you would take 60mg np thyroid which is what I'm taking. It goes on to say that 60mg np thyroid contains 38mcg t4 and 9mcg t3

bogus labs. Re-test. Ypu don`t take thyroid meds the morning of labs. Would you take a shot of T 2 hours before a test? Aiming to suppress TSH down below 1 is stupid and not thyroid replacement.

No, not replacement. optimization. Similar to trt+. Then again plenty of people think that's stupid too. It's okay if you don't like it brother. I'm just trying it out and testing different doses to see what I notice. It's only $10/month. If I don't like it I'll just drop it
I'll retest first thing in morning before I take it next time

The t4 in it will be longer acting yeah, but not the t3 as far as I know. I’d say you’re definitely far from catabolic territory so I wouldn’t stress that. How are you feeling symptoms wise? No hypo symptoms?

You don’t have to skip it, you just have to be aware of how it’ll reflect in your labs.

Optimization? LOL. With the goal of staying leaner? Tighten up your food and training before wrecking your thyroid. Everyone looking for the ultimate drug shortcut, bio hack they saw on IG. UGH

Oh my bad, EVERYTHING on youtube is legit and vetted. 1 minute in and I`m done with hose hacks. "the 2 biggest feel good hormones are test and thyroid"... GEEZ!! No mention of tanked cortisol and how that makes you feel from being over stressed?! Cortisol is more often out of whack than thyroid, unless you have tried to "optimize" it.
