How do I avoid this type of Injury?

In the last 2 months I have started switching from machine weights to free weights, with squats being a big part of my routine and I keep getting stronger and stronger, it's insane. I saw this video and worry that I may do this also and want to avoid it. Any tips on what he did wrong so I can avoid it?

Probably just avoid going for a 1rp max

I had a friend who snapped his tendon on an easy 5th or 6 th rep of a set he was intending to do 15 reps with. I think it has more to do with certain drugs making the tendons and ligaments brittle and unable to handle the same load the actual muscle can.
 
At 44, this is also a concern for myself. Having trained heavy till a couple of years ago, it took some willpower to lift the foot off the pedal...thankfully self preservation won out.

For me-

1. Good warm up
2. Feel the weight and focus on the squeeze ( guess I knew this but never REALLY applied it)
3. I like pre exhaustion
4. I use dumbbells on most movements- even on squats now sometimes- Bulgarian split squats =awesome
4. Higher reps- took some getting use it this, but now love it
5. If you still need to go heavy- cycle it with something like GVT
 
Long as you warm up and keep good form you should be all good my man

Both quad tendons snapped.

I do not want to sound like a dick, but: I read that the lifter missed his lighter opener, then decided to go heavier on the second attempt. Always risky. IMO, he never had control of the weight, his hips were visibly moving and not stable. Just too much weight. Again, just IMO, based on working with powerlifters for a few years.

Even if he had done that weight before, he clearly was not ready for it on that day. Listen to your body, make your attempt selections with your brain, not your ego.
 
Be like most the guys at my gym and dont train legs :D

My body and joints cant take the beating they used to, so like a previous poster mentioned, pre-exhaustion. I normally do this when I train legs and havent had any issued with growth or developing them going this route
