Kidney issues-the whole story

If I am allowed to participate in posts and post links to articles and leave my signature with links I'll still be here. I wrote to Big A asking about the rules.

To that really smart guy who asked about liver,kidney,I have answered the questions in my blog and anything I didn't answer just eme [email protected] and that goes for anyone here.

I would presume that you use creatinine, eGFR and BUN as a primary evaluation of your weight training clients renal function?

Do you use any other Assays or makers for assessment?

Yes,corect on initial assessment. But I always like to know if someone is dumping protein or (eek!) blood in their urine even if KFT's are normal so I always get a UA and micro.If there is protein it can be tubular (usually a BP issue or a glucose issue) or glomerular (what we really don't want to see).....so I keep that in mind when assessing kidneys.

what blood pressure medication would you recommend?



im currently taking lisinopril.. it's said to directly protect glomeruli in addition to lowering blood pressure (though the mechanism is not necessarily understood).


would you recommend telmisartan over lisinopril? im speaking strictly regarding kidney benefits.

HI!\
Lisinopril is an Angiotension converting enzyme inhibitor so it is most likely a very good BP med for someone who is a bodybuilder. It does inhihibit progressioin of glomerular disease.

But there is something I would ask you to do:
1. Look at the thread on cholesterol-IDK what anabolics you are on or not but if you can do some mix and matching that would help your kidneys.

2. I would max out on NO enhancers which relax blood vessels and help BP. By definition if you have BP issues you are NO deficient. I have a thread on this as well.

Does Cialis relax blood vessals just like NO supplements? Ive read many times that it lowers BP but i have seen a few others say it doesnt
 
Good thread on kidney issues.I'd say the majority of the guys cycling steroids frequently and in high doses have a good chance of developing focal segmental glomerulosclerosis.

Development of Focal Segmental Glomerulosclerosis after Anabolic Steroid Abuse

I had almost the same profile as the bodybuilder in the article, kidney function up and down when on cycles like we normally do.
Luckily I am stable for now and will continue to be since I don't cycle anymore, but was a see saw for many years because of this.

I wonder if those guys had untreated OSA, hypertension, runaway blood glucose, absurd levels of inflammation, any genetic predispositions...etc..

While its almost always multifactorial I do personally believe that AAS at higher doses will directly contribute to FSGN. In 5 years i have seen it twice "in the wild" meaning I was directly involved with the biopsy and spoke with the pathologist doing it and the patient ( lots of AAS users at my prior jobs patient population). I have also talked to a handful of guys on the boards who were ultimately diagnosed with it.

There is probably other things that contributed but ultimately i do think AAS have a direct effect.

As i frequently discuss for cardioprotective reasons i mention use of ACEI or ARBS i do feel that those will help decrease some renal issues ( no i cant prove this as im just speculating) associated with AAS. But it comes down to keeping BP under control, hydration, not taking large doses for extended peroids of time.

Let me answer a couple of things here:
to OutToLunch-correct-what was mentioned was liver support and I have the BEST one.........

Kidney support is just drinking water until your pee is clear, don't take diuretics and here is my idea for anabolics....

Have you ever tried a cycle of IGF1LR3 which increases musculoskeltal UNITS not just hypertrophies them, increases them? This (which Toraxx will have soon) plus good (avoiding auto link on site here) BCAAA-l-glutaminee which I have actually equals (I know it's hard to believe but it does) anabolic gains. It would at LEAST allow you to cut down anabolic doses.

If you protect liver, treat inflammation, oxidative stress, set up an environment where you cannot get heart disease and watch eGFR then cycling on and off anabolics can be made not all that harmful.

That's why I've invited everyone to do a consult and then get followup with me for a year for only about 80 bucks.

I have a friend who is in stage 3 kindney failure; anything he can take you recommend? He has never taken AAS in his life; his failure is genetic. He said he started taking magnesium and it has boosted his energy levels.

Oh wow,very sorry to hear that...

He is likely being monitored by his doctor and all electrolyes need to be watched, especially K and be careful with Mg dosing-make sure he tells his doc.

He is probably on a protein restricted diet already...

Regular docs don't check hormones but if he's T deficient that sure would make him feel better.

Good luck to him...hope he's "on the list"-people can live long lives with a transplant.....

My Kidney doc rates me a border 2-3 stage CKD, my GFR is 59-60. He has not altered my diet at all and he is doesn't seem too concerned other than keeping an eye on it. I am on amlodipine and Quinapril for my BP, he just raised my Quinapril from 10mgs to 20 mgs to get BP under control.

So a 24 hour urine to see how much protein you are spilling is mandatory (if a lot then protein restriction prevents progression) and a UA microscopic to see if there are RBC's (I'm sure this was done?) is also mandatory.

2. Depending on your age you may have a NORMAL eGFR....maybe you are really young....

3. Make sure you have a good BP cuff at home and keep your BP normal

4. Finally even though you are on BP med,by definition,when someone has HTN,they have ongoing endothelial damage and low NO levels. So you really need a 2x/day good NO enhancer-not just garcillin. OR...the BP just keeps going up and up and up...just did a consult with one of the MODS and he is on tons of BP meds because HE didn't supplement with NO.

I am 49 yo, competed for 9 years and I have supplemented with AAS for 30 years. Looking at my blood work over the last several years, my eGFR was as low as 49 in 2011, to 59-60 this year. I was first diagnosed with high bp in 2011 and went on the meds then, stayed on them but haven't been getting checked regularly by a hypertension doctor, just my GP.
Back in 2011, I did do a 24 hour urine test. result then were 2599.2 on the 24 hr creatinine; 4756.7 on the protein total 24 hr. I had an ultrasound in 2011 and again this january and both, I was told were normal.
Under my latest blood work, my urine test showed 2+ on the protein, this has been 2-3 for several years, you mention RBCs it shows 0-2 in the urine.
I am not sure what you mean by NO.
One thing to note, my Kappa/lambda ratio was 2.80 which, once I found out what it was, caused concern, but he mentioned this was not too much of a concern as my other numbers were ok. I did read that this can be cause for multiple myloma too so I went to a cancer specialist to monitor.

This post is much appreciated. Wondering if I have cause for concern. I'm a 5'8" 220lbs male with low body fat...carry a good amount of muscle mass. Had recent lab test that read as follows:

BUN-20
Creatinine-1.1
eGFR-95
Bun/Creatinine ratio--19
Albumin-4.3

All within range. Historically all these numbers have been in range in previous test.

That said, I went to the doctor this time with a low grade fever (100.0), and for the first time ever had high blood pressure readings...either because of the virus I was combating or an anabolic I was utilizing. Usually my BP is very low...120/60...this time it was 140/100. That said a random urinalysis showed a micro albumin urine concentration of 37.5 which is high on a scale of 0-20.

Given the past history of solid aforementioned parameters and the fact that high BP/fever can cause acute microalbuminuria, do you think I should let this single test keep me up at night?
