Labs A1c/glucose insulin resistance

Had my labs done a few weeks ago been awhile. I had not been on any meds for awhile for Type 2. Had some tragedy's in my family and personal life PTSD just jacked in the head not eating right. But it was a wake up call.

All the other labs were good.

I have changed to a strict diet like I used. I don't fit the profile at all lean/muscular Doc says 1000mg a day of metformin 5mg of glipzeride labs in 3 months. Insulin resistant at 3.2 based off division factor. Going back to the gym in a few weeks. 54

Holy shit dude. You need to fix that immediately. Those numbers are horrific

Drop the carbs (a lot), up the cardio (a lot), I’d even add in a glp-1 like tirzepatide and maybe even add berberine. Your numbers aren’t just out of range, you’re in the extreme end of being completely off the charts
 
Yes sir I know super bad. I was looking at the tirzepatide to help I do have berberine. Would need to figure out the dosing protocal. I appreciate the idea's. I was eating reese's cups and carb over load majorly.
 
I can only imagine the amount of damage you’ve done to your body with levels in that range. That’s insane dude
 
I'd be careful and check with doc about adding other drugs to metformin. Low carb, lower cal, fasted cardio, etc

I mean most people start to worry when blood glucose is consistently over 100 (fasted). But 382!? Jesus fuck dude

are you a type 1 diabetic? otherwise it is practically impossible to have such bad results
 
Neurocognitive impairment and nephropathy doesn't sound too appealing to me. How about you?

Skip the berberine and go for the pharmaceutical route.

I'm suprised your Doc didn't recommend 1000mg of Metformin twice a day. Add in a glp-1 like tirzepatide for sure as well.

My mother was type 1. Her glucose on occasion would be close to 600. Sadly she didn't take very good care of herself.

I would consider discussing the addition of a GLP with him until you can get back on track with your diet, or better put to help you do so.

Nothing ends well with those kind of numbers as others have pointed out.

What do you think the best option for OP would be in this case? I mean ontop of dieting, cutting carbs, and doing cardio.

Do you think he should take a semi large dose of insulin to get his blood sugar down (even though he’s insulin resistant), than once he gets it down to 100-150, lets the diet, cardio, and supplements carry the rest? I feel like the biggest priority should be getting the blood sugar down as soon as possible (safely) but I’m not a medical professional, so that’s why I’m wondering what you think

I’m genuinely curious what the best approach would be here because I’ve never seen these levels before

Glipizide IR/Metformin should suffice at getting his glucose under control. Insulin would work more rapidly. In his case, I wouldn't see it to be necessary. If his glu was 500-700+ mg/dL. A bolus dose of insulin at first then transition to the aforementioned combination. Basically what he said his Dr.rxd.

if i was in your shoes I would check fingerstick glucose atleast 3 to 4 times a day. Fasting, Pre meal, post meal etc. May even consider a continuous glucosemeters that you attach to your arm.

Then if the glucoses are not rapidly improving to an acceptable level within 10 days or so of being on meds, I would reach out to doc again and see if he/she wants to add something or refer you to an endo. GLP1s have had very good results.

There is a chance that metformin and glipzeride may not be enough for someone with your level of insulin resistance.
 
that is not true. plenty of uncontrolled type 2 diabetics with a1c of 14 . usually ppl that are non compliant and dont care
So I am really shocked what to do and how to neglect myself to get to such bad a state
