Night Sweats smell like Vinegar

Title says it all. When I sweat during the day I don't stink. Only at night. Smells like vinegar.
This has been going on for years.

Diet is super clean last 7 weeks chicken breast, jasmine rice, cream of rice, broccoli, and Greek yogurt. I eat the same thing everyday.

I know it's not the gear because this happens whether I'm on 200mg test or 2g gear total.

This is the only thing I can think that might be a culprit: Creatine

When I take Creatine even small doses of 3g a day, my kidneys hurt. I got blood work done and all looked good except for high ldl and low hdl and slightly elevated creatinine.

Blood sugar is good. I'm not diabetic.

If this happened when I sweat during the day I'd just say hey I guess I fucking stink, but only at night.... ???

Any thoughts?
The vinegar smell at night could be due to excess acetic acid buildup from protein breakdown or metabolic stress, especially if your diet is high in protein and carbs. Elevated creatinine and kidney discomfort from creatine might also play a role. Try cutting creatine for a few weeks and rotating some foods. If it persists, a deeper look into your metabolism or sweat glands might help.

Only thing that stands out, not overly concerning is your osmolality (hydration status). Its slightly out of <range, which most likely you're taking in a little too much fluid. Your BUN, NL:R, albumin and protein looks good. Your >creatinine, could very well be from supplemental creatine, which is very common as it's a metabolite of creatine.

As for remedies... it may be diet related as @btails pointed out or a slightly sluggish urea cycle. You could try citrulline to see if that might help. If it continues to be bothersome, I wouldn't hesitate to keep mentioning something to your treating clinican.

It's a relatively inexpensive test. Your liver function test doesn't indicate hepatic injury. Although, I wouldn't detract from teasing out this simple test.

There's a few other things that you may try if supplementing with citrulline doesn't alleviate the funk smelling odor.

Getting ready to hit some cardio. If wanted, I can throw some more ideas out there. No guarantee that these applications will subside this ongoing issue. Moreover, pondering thoughts.
 
@aminofreak you need to significantly increase your magnesium intake based on your labs. I would increase hydration and add electrolytes as well.

His osmolality is 282.7 mOsm/kg, slightly on the lower end of OOR, and his albumin are tell-tell signs of his hydration status, which indicates too much hydration. This will bastardize the solutes concentrations -->electrolytes in blood. That's if there's no other pathological causes.

Out of curiosity, genuine curiosity. How are you determining he needs to up his Mg significantly? There's no differential diagnosis on his labs (CBC/chem panel) that indicates his RBC Mg status.

Low alkaline phosphatase could indicate low magnesium in the tissue. But you are correct based on the other markers as I only did a quick glance.

I most certainly don't disagree that Mg deficiency is a marker to further assess RBC Mg. Unfortunately with a <alkaline phosphatase, it's a non-specific marker. It's related to a few different vitamin/mineral deficiencies. Zinc being one of those, in-turn, could cause funky smelling body odor, such as the smell he's describing- vinegar. As for other <alkaline phosphatase, there's a laundry list of other potential causes. It's non specific, unfortunately. Good thinking otherwise.

Agreed. I would want to run RBC magnesium, zinc and a full thyroid panel based on the low alkaline phosphatase and excess sweating at night if it were me personally.

The zinc would be more likely related to the smell as you said.
