partial quad tendon tear at knee

a month ago i banged my knee on a metal pole, right at the top, very hard,,, the next day, could barley move it

an x-ray showed nothing,they said contusion, didn't get better after a week, so they sent me to PT

PT thought the same, had do work it out, squats (i can barley bend it much without pain), stuff strengthen the quad

after a month of not getting better or worse, i got an MRI< which says...

meniscus intact
mild edema in acl, this may be from previous injury or synovitis
pcl is intact
increased t2 signal edema within distal quad tendon consistent with tendinosis/partial tear. complex fluid collection anterior to the upper pole of the patella slightly lateral extending to the distalmost quad fibers laterally. collection most likely represents a hematoma and measure 2.4cm transverse x 1.6cm ap x 2.3cm craniocaudad. there is rather extensive adjacent subcutaeneous soft tissue edema

lcl complext intact. patellar tendon intact
mcl intact
small amount of knee joint fluid. no significant bakers cyst.
small cartilage defect on lateral facet of patella



my PT said it doesn't say how big the tear is, a knee specialist will determine if i need surgery, or simply PT to strengthen it back to normal.

Hmmmm, any thoughts?

Thats a really poor report....


From the way they described it you looks like you have some tendinosis but no full thickness tear. You may have avulsed a few fibers from the insertion of the vastus lat and vastus int from its normal patella insertion but nothing major

they didnt mention the lateral patella fem ligament... that is important based on the findings.

the lateral pateall facet cart defect is important and they dont say if its high grade or not.. may need to keep an eye on that.

What i bet happens is banged knee on pole... patella impacted lat fem trochlea. Caused cartilage injury. At same time it resulted in avulsion of a few tendinotic fibers from its normal lateral insertion and the combo caused a small hematoma.

Nothing you are really gonna do for it. It take a month or so off and let it heal based on the info you have. Leg workouts and PT will make it worse in the short term.

There ya go.. saved you a trip to the "knee specialist"

thanks for your help, much appreciated.

i am going to see the surgeon so he can actually look at the MRI, and tell me what percentage tear it is (a few fibers like you said, or if it is a larger tear).

its 4 weeks later, i can lift my lower leg (like on the machine at the gym, leg raise working quad), but its quite painful, and difficult. I can bend the knee and walk without a limp when heating it, but have trouble squatting down on it.

Do you think im still recovering from the contusion, or its more the "tear"?

Thanks for your help, im know you can't tell much based on me typing the report but any information helps, i will be seeing the surgeon Monday, and he is supposed to be a good one.
