Besides the CPK as mentioned already I probably would be most concerned about the panic attacks (and worrying about that will probably make your anxiety worse). The 28 HDL is not surprising considering the cycle so I would focus on getting that dropped closer to or under 100. Trigs are low so that's good.
I think anyone running 3.7g per week is going to have some issues if they are prone to panic attacks and/or anxiety. You really need to get that in check especially if you are going to run heavy cycles like that. I know it sounds gay but I'd try breathing exercises, meditation, etc to try to control things better. And yes of course lowering the gear will help.
