Posture Excersizes: shoulders

Most likely your chest over powers your upper back so make sure for every bench you do a row, shoulder press you do a chin etc. also make sure you train your rear delts. Stretching your chest will also help you stop rolling your shoulders.

if you have access to a foam roller you can do abs on it, and it will put you in a position on the negative portion to better your posture. make sure the foam roller is just below your shoulder blades, and keep you hands touching your ears and elbows together. keep your feet and butt on the ground, in the same position as a sit/crunch. try to touch the back of your head to the floor each time you come back.
 
As Braven said, it's most likely stemming from tightness in your chest and shoulders. Check out DC's extreme stretching along with regular massage work(thai) and daily stretching
 
Good hard seated cable rows with perfect form and hold the squeeze for a two count at the back. Also one arm d/bell rows! Rear delt work is a must. All these need to be executed with perfect form and hit the squeeze on each real hard.
