22 year old female athlete

Here is what is going on with me---

I all the sudden developed really bad edema all over my body. It is worse in my legs. I noticed for the first time about a month ago after a workout, I weighed myself before then after and noticed I had gain a rather unusually matter of weight. I thought this is odd sense I just worked out. I know I did cardio workout, stair climber. My legs felt weird and my muscles. I didn’t do anything about it thought I would just let it go. So then I didn’t really workout much after that. I do remember if I did it would be running miles at a time and ab workouts. I still noticed that my weight was increasing for some weird reason and didn’t understand why. I felt like my body was retaining fluid, especially in my legs and stomach. I had changed nothing in my diet and nothing else about my daily living.

The only thing that I know that change is I started an accelerated nursing program that is really intense and hard. I know I have never ever been more stress before in my life.

I saw myself gaining weight sudden, within a matter of days at a time. I couldn’t understand this. There was a period where I gained 8lbs in a matter of three days.( typically weigh 121 and now, to this day, up to 129-130) I looked at my diet and tried everything nothing seemed to work. Another thing to be noted I had a TB testing on 7-1-2010 and first Hep A shot on 6-28-2010.

It peaked, the edema that is, whenever I was at clinical’s. I was on my feet and notice the edema was getting worse and worse. I told my instructor and showed her and was told to go to ER.

1 visit- did a venous Doppler and found nothing. They thought blood clots, or some type of an assist growing. But nothing

2 visit to ER did blood work, found high liver enzymes, slightly. Then did Hepatitis testing and found that I do not have Hepatitis. They didn’t know. Got nothing

3 visit went to primary care doctor. He knows I have retaining of fluid and thought maybe I had a reaction to something. So I got a steroid shot and a diatriect. I also got an ECHO on my heart where nothing was shown. I told my doctor it seem as though the swelling got worse after I workout and he said well that just doesn’t make sense, and that was all. I saw a little improvement but not much. I wasn’t able to work out for a while, probably took about 4weeks off, and saw a little of the weight off but nothing back to my normal range. He told me I was able to go back to work out since nothing was wrong with my heart. So, being the crazy workout person I am, I went straight to gym and did cardio and lifted. I lifted a little bit of everything legs, arms, and back. I also did ab workout. That’s when I noticed the swelling coming back. The next day it was worse, I had pain really bad in my legs ( quads, hamstrings, calfs). The back was sore. My ab’s felts hard and distended. The legs were the worse. I called the doctor and made an appointment. He saw the swelling in my legs and got a steroid drug, and pain medication. That helped a little, the pain for sure was gone but the swelling was not. I was also on break from school, with no stress. So I was able to go and workout, and so then I went and then it happened again, I workout and I felt swollen afterwards and the next day still, maybe, worse of edema.

Went to doctor again because the swelling was not better, and I went back to school. I went running one day got cramps (in muscles) when running. Then felt swollen afterwards and stomach felt swollen as well. He got blood work again and some other test to check for autoimmune or something. Tests showed nothing, think liver was still high, not sure though.

Throughout this period I have changed nothing in my diet. I stayed the same and was counting calories and trying to figure out as to why I was gaining all this weight. I notice that some days I was getting around 1090-1200. I thought there is no way I could be gaining weight. My diet—

Pure Protein bars (2), Almonds 100 cal packs (2), Protein shake (think ESA) and Muscle milk light 100 cal. Eat Oatmeal 1-2serving a day (plain). Sweet potatoes. Vegetables. Egg whites. Chicken breast. Tuna low sodium. Apples. Lettuce. I drink water mainly. May have vitamin water flavored zero cal. Fish. (EAT every 2-3hrs)

The above is what I would have on most days.

Some days I would have yogurt, soy milk, all natural peanut butter with flex seed.

So lately I have tried and switch it up to try and change the weight gain. I increased more fruits and vegatables. But have eaten less way less.

Last visit to doctor was ER, they found nothing in blood work again. Said it was stress related and wrote me a prescription for anxiety.

The only other medical condition I have is IBS. I control this with diet and exercise.

To this day swelling is still occurring in legs (worse) and stomach and arms.

Aug 27 weigh 130, Aug 28- 131, Aug 29-132

The only thing they found in Blood was high liver enzymes which were—AST-81 and ALT-120

Normal for AST (5 to 40 units per liter of serum).

Normal for ALT (7-56 units per liter of serum).

BUN is normal, CK unchecked,

shes been under a lot of stress lately but seems like it is more than stress.

any ideas? I did some looking into those oregon football players because I saw some similarities but I'm not convinced thats what it could be.

Are you on the pill? Did you start taking it at about the same time the swelling appeared? Did you have you estro/progesterone levels checked lately?

First thought would be kidney disease, but it'd have shown in your bloodwork...

Cellulitis (spelling?) comes to mind except that it doesn't usually present bi-laterally. The fact that her belly is also retaining fluid is alarming. You've ruled out congestive heart failure. Is she taking a diuretic? she probably should be. Is she currently on a cycle? Is she pregnant? Is she urinating frequently or average amounts? You said her blood sugar levels were okay? What is her water consumption daily? Not drinking enough will cause the body to try to retain fluid. Lots of questions, but will help.

Heavy duty issues for this site.

Sounds a little like she is over dosing on cortico-steroids. I was a product marketing manager for mythlyprednisolone. I sure as hell am no doctor, but it seems odd to me to give a corticosteriod shot for edema. They cause edema. But wtf do i know? My best wishes on her speedy return to good health.

I would recommend an endocrinologist. You need a specialist. The doctor's keep repeating the same tests and gettin nowhere. That is the reality of medicine.

The elevated liver enzymes are of concern as there doesn't seem to be any reason for this.

Do you drink alcohol, use other prescription medications, pregnant?, high BP?

Get to a specialist!! The ER is for emergency medicine. They will do nothing for this...

alcohol yes not to excess, average college consumption
no drugs, no pregnancy, no birth control

told her to go see an endo tomorrow

her GP told her no need to test for hormones and that he thinks this is all "stress" from school.
