Hi guys maybe a strange one and a long shot.

My missus has competed in the past, since having our child 2 years ago she has suffered with rheumatoid arthritis, she has flare ups when she has difficulty with her joints etc, she is currently taking methotrexate to manage it.

Do any of you have any experience with this, is there any other supplements she couldn’t that are affective, she has used gh and anavar in the past, would these be beneficial, is there anything else she could take that would be better, she would like to compete again but due to her condition she is often in pain during flare ups or tired due to the medication and often stops taking it (probably not a good idea) it’s really got her down, any help would be massively appreciated by myself and her

If you haven't already, have her vitamin D levels checked. There are many studies on this, but to give an example, here's one from the 1930's that was researching high doses of vitamin D for hay fever and coincidentally were surprised by significant improvements to RA:

The treatment of arthritis with massive doses of vitamin D

"the usual dose was 300,000 to 500,000 units daily"

I'm quoting this from that study just to illustrate how much vitamin D people can generally tolerate, not to recommend such dosing. Testing is imperative, both initial and ongoing (n.b., for ongoing tests, discontinue vitamin D for four weeks before the test). You'll find a lot of solid studies on vitamin D and auto-immunity in general. I think doing labs for this is also appropriate given the mention of fatigue.

Separately, tirzepatide is especially effective at addressing inflammation. I know someone with RA (stage III) who saw interleukin levels go from off-the-charts to low-normal on just 2.5mg SQ/week. There was more than just the improvement in labs, pain and stiffness completely resolved. And I'm not talking about knee pain resolving after losing 300 lbs.

Lastly, look into Dr. Longo's Fasting Mimicking Diet (marketed as Prolon) and his book The Longevity Diet. Don't let the title dissuade you, it's a good read and he covers auto-immunity extensively. I mention this one last because you say she's not always compliant with medication, so she may not be up for five days of deep caloric restriction.

Focus heavily on gut health and inflammation management. The key for her will be to keep the inflammation in check and her immune system balanced.

I’m personally a fan of using aged garlic extract with each meal in capsule form, MSM 4-5g daily and proteolytic enzymes empty stomach AM. Using 20g of beef collagen daily will also go a long ways. I also agree with the vitamin d suggestion above. 5000-10000 iu daily based on bloodwork would be my suggestion.

Electrolytes and minerals such as magnesium along with proper hydration are a foundation as I am sure you know.

If the flare ups are frequently common I personally would consider hydroxychloroquine before methotrexate, but I don’t know her full history and sure her doctor has their reason for using it.

You could also consider and explore LDN as an option as well.

There are bigger plays such as stem cell therapy which have been very successful, but overall this just becomes a lifestyle in terms of management.

Whether or not it’ll make a notable difference no one can say blanketly, but it won’t hurt.

I would consider using oral KPV in this case.

100% agree.

Gut Health is numero uno.

I just started LDN recently for inflammation, not RA, but LDN is indicated as a treatment for RA and it's pretty amazing how quickly it begins to work for my issues.

Get a Microbiome/Gut Map done and a consult to begin fixing digestive health. Firmly believe this focus would put a lot of autoimmune and other common issues into complete remission or significant improvement for the large majority of the population in the US. We eat terrible foods with little diversity and destroy our guts which is the source of almost all disease. Especially modern day diseases and the inflammation which cause them.

Not without fixing your nutrition and gut health. I've taken KPV, BPC, TB, etc etc. Those things imo only help speed the process along when you are actively healing your gut through focused attention on sleep, stress, recovery and getting the proper testing done to know how to fix and diversify your diet while idnetifying your specific gut issues.. SIBO, parasites, etc.

Otherwise you will likely spend a lot of money on peptides that won't do anything at all or certainly won't do anything long-term to fix the problem.

And I have also given some of these peptides to a friend with UC... didn't help him at all because he didn't do the rest of what I told him he needed to do.
 
You might also consider undenatured type 2 collagen. This isn't used to rebuild connective tissue or anything like that. Rather to train your immune system not to attack cartilage.

So, I'm big dumb and can't really give a deep dive, but my understanding is that uc-ii is very similar to our cartilage. Immune cells in our gut get exposed and recognize the collagen isn't harmful and your immune system "learns" to stop attacking your joints.

Or something like that lol. Anecdotally, I started using it last November and my joints feel better than ever. I started using meriva at the same time. So maybe that's it... Or both. Obviously it would have been better to do one at a time but tbh I never really expected either to make a noticeable difference.

I personally am not a huge fan of them as they carry a lot of potential risk and side effects which could lead to one getting very sick due to the lowered immune system response. I think there are better options, but that’s not to say that some don’t get relief from them.
