Self Prescribed TRT Question

Trying to help out a friend. He recently started “self prescribed TRT”, I recommend my HRT Doc but unfortunately he doesn’t have the $$ and his insurance won’t cover anything. Testosterone levels were >300, estradiol around 70-80. Big dude, estimated 30-40% body fat. Any idea starting Arimidex dose before getting bloodwork in 4-8 weeks?

He’s starting at 120mg Test Cyp 1x week.

Does he plan on loosing weight? Fat has aromatase enzyme that converts T to E2. I would get my T levels checked and stabilized before adding another variable like arimidex.

At that low dose he should not begin with any at all. AI is best avoided until it is confirmed to be absolutely required. For me it is unless I subq EOD.

Thats a high estrogen reading natty, no doubt will it increase once he starts testosterone. I'd advise at least 0.25 twice a week, but he could always do none and wait till week 4 but if he gets gyno symptoms early, dose asap.

probably not popular but my recommendation would be to get his BF down to around 12% and retest, hold off on trt.

Sorry for late reply. Absolutely, first thing I said is to lose weight asap. Get to that 10-15% bf range but with his starting point being so high it’s going to take time to get there I assume. He started a week ago with no AI and will be getting bloods in 7 weeks. Imo seems like he’s setting himself up for bad gyno but he was pretty desperate to start and I’m not really knowledgeable when it comes to AI dosages especially at his very high bf%.

A high body fat at a real TRT dose does not usually cause any gyno issues with the vast majority from what i have seen in my 20+ years on TRT.
