I'm up at 4:20 am, and usually at the gym by 5:00 am. I have never really had anything to eat before going, but worried that I may be hindering my efforts by training in a fasted state.

A post-workout shake is a given, but how many will have something before if your work out starts within 30 minutes of waking up?

I used to train at six am when i was power lifting. I did not eat prior to training and just had a coffee and went. I did post up some big numbers on an empty stomach. You can eat some carbs the night before and that will help with energy as the carbs will be stored as glycogen in the muscle. You will need to eat a substantial breakfast as soon as possible after training though, lots or protein and carbs to replenish.
 
I always eat after, reason is when you eat blood goes to the
stomach area during digestion which you dont want. You want
as much blood pumped to the muscle as pos. coffee in the mornin
and that's it for me.
