So many of my friends and fellow weightlifters suffer from sinus congestion.Now like always its best to understand what is going on before treating. However, not everyone can just go out and get a CT scan and i understand that. So id like to discuss what can be done without imaging.

Please note: This is not a replacement for advice from your ENT or in anyway a reason to ignore more serious symptoms. Im also not suggesting you do everything mentioned as it includes prescription medications and may have sides.

Many bodybuilders suffer from breathing at night. Some of it is likely obstructive sleep apnea at the neck level BUT some if it could be straight up messed up turbinates that swell while you lay down.

I personally had this issue. My turbinates were messed up big time for years along with slight nasal deviation. I chose to have submucosal resection and then follow up coblation.

A quick way to see if this is an issue is to see if your nighttime nasal congestion responds to a good dose of afrin. Afrin will constrict your blood vessels resulting in shrinkage of your turbinates. If you can breathe acceptably well at night after taking a hit of afrin then continue reading. if not then its likely more structural.

Prior to considering turbinate surgery id suggest the following

1. nasal steroids
2. nasal irrigation
3. Short dose of oral steroids ( noo..not dbol...im taking like prednisone)



Nasal steroids

There will directly bring down the local inflammation but its only going to help so much if you dont stop them from getting reinflammed. Id suggest somthing like flonase 2 sprays each nostril 2x daily.

What i personaly like to use along with a nasal steroid is azelastine. Works localy on histamine. its prescription but works quite well and also has antiinflam properties.

if that doesnt work... well.. use with caution but what worked well for me and other physicians who suggested i try it is is mix 4mg of dexamethasone into some saline spray and use that for a few days. that works very well! Now some is gonna go systemic so treat like oral but since its a nonparticulate steroid i think some gets absorbed into the mucosa. I use this to get turbinates back down to size when neeeded.

Most physicians wont do direct steroid injections..blindness blah blah..its overstated but whatever.

Nasal irrigation

Ok...this is needed! I do this once every day and will continue to. I dont care about some studies showing decrease ciliary activity with hypertonic saline. The benefits for me are worth it.

You have two options. Hypertonic saline or hyper plus a surfactant like baby shampoo. I do the 2nd.

I use a 20 cc syringe, add some salt to a cup , two squirts of baby shampoo and some warm water. Suck up water and gently irrigate sinuses.

the baby shampoo is very important. Lots of nasal bacteria make biofilms that are bodies and oral antibiotics cant clear. this happened to me. despite high dose antibiotics i could never full kick the infections.....biofilms! Surfactants like baby shampoo help clear those so our bodies can kick its ass.

Last option is oral meds like steroids and antibiotics.

sometimes you have to bring down the inflammation big time to get sinuses to drain and so other things like irrigations and flonoase can work. Hit your sinuses with a medrol dose pack and possibly in combination with some antibiotics. At same time make sure you are doing the prior mentioned things.


IF the afrin spray helped you they above steps should also help to a degree. DO NOT continue to use afrin spray for more then 3 days. If however you did get hooked then id suggest diluting the spray to cut down, using only at night. Taking bronkaid during the day to help and you may need medrol dose pack during your time complete off afrin to help.


If the above doesnt adequetly help then look into submucosal resection. Much better long term results with SMR over coblation.


And since i feel compelled to mention this like 9x... this is not a substitute for advice from your physician. Just a few things to consider which may be useful or can discuss with your physician.

I lately have been hit with this and actually have an appointment with an ENT at end of month because not only is it affecting my sleep now but on my heavy lifts I am coughing up shit that have no idea where it's even coming from, get stuffed up, feel far more constricted in my airways than I ever did previously in my life and flat out have a hard time breathing to the point where my anxiety shoots through the roof as I feel like I can't get enough air in me. It's not in my head either. At night I feel like it's not only congested airway, but phlegm lodged right in the back of my throat where I can't seem to get at it and it constricts my airway, at least it feels like it. Never had this issue before, but it is definitely beginning to give me cause for concern. I just turned 50 so figured it couldn't hurt to get it checked out and not assume it will pass...it's been a few months now. When it wakes me up at night I often can't go back to sleep and have to stand upright or sit straight up until my heart rate goes down and I feel like i can breath normally again. Not a good feeling.

I was the same way. I would wake up short of breathe, i used 4 pillows to stay upright. Had have post nasal drip because nothing could go forward. Id sleep only 5 hours a night because id rather be tired then lay down.

This went on for years. Even after my sinus surgery it all came back again. I wanted to see what was going on and cultured my sinuses. Had an infection and treated it appropriately with the correct antibiotics which along with everything else helped...but then it came back again!

it wasnt until i added in the azelastine daily and added baby shampoo to my irrigations that i saw real improvements.

Swollen inflammed turbinates are VERY common. Id feel the best when i was active in the gym, maybe popped a few bronkaid and doing cardio.. Then id stop and relax at home in the evening and couldnt breathe at all. Its a scary feeling waking up at night gasping for air or snoring so badly the hot blonde polish model next to you is like Dr GG...maybe you should see someone about that lol.


I am 95% perfect now. Low dose nasal spray to keep things in check and daily sinus flushes.

As for that sinus irrigation link... id stick with a 20cc syringe just be gentle. Not trying to power wash your sinuses.

I suffer (a bit) from nasal congestion. And I pretty
much know where it comes from . . . a neighbors
cat that adopted us. He is here day and night,
never goes home, sleeps on the bed, bla, bla, bla.
Great cat but damn, I think I am allergic to him
(have had cat allergies in the past).

That being sad, the cat is staying so I am dealing
some nasal congestion. Two things have helped
me, and it’s mostly sleep related, slight congestion
at night so I use a Neti pot (follow the directions)
and nasal sprays called Aller-Flo (Kirkland) and
Flonase. Both have been ‘okayed’ by my ENT doc.

Just my 0.02 cents worth . . .

I have obstructive sleep apnea and where a full face mask because I'm a mouth-breather. The last time I went to the doc I was told I had a severely deviated septum and that I COULD correct that...but that it wasn't a 100% chance that it would fix my nasal congestion.

As of right now, I basically LIVE on Afrin, which I know isn't good.
First step is getting some Flonase tonight then.

If you sinus congestion was almost entirely due to your septum then afrin wouldnt help much. If you feel that your breathing is acceptable after using afrin then look into turbinate issues.

Chronic afrin use could result in a situation making turbinate reduction basically necessary after having resulting hypertrophy

I was hooked on Afrin for over 20 years. Lost count of how long. I finally go off about a year ago. What worked for me was this, got it off a youtube video you can probably find if you search:

1. FIrst week continue using the Afrin like you normally do. Take two sprays in each nostril of Flonase each day. I did it at night before bed. So you are using both meds at the same time.
2, week 2. Keep on using the afrin like you normally do, but each morning fill the afrin bottle to full by putting in saline spray. Just buy some generic saline(salt water) spray for your nose. So each day you use the afrin spray, you are getting less and less of the actual med and more of the saline.
3. Within a week or two you will be spraying in nothing but saline, no Afrin at all!
4. After that is done, if youre like me, you can most of the time get by on just one spray of Flonase per nostril. I never thought this would work for me, but it did!

Try that out and let us know if it worked. I still use saline spray, and if I spray that in my nose now at night before bed it opens my nose up. I spray that first and then a few minutes later I spray the Flonase.

Thank you for this, I wish I could have had this information months ago. Yes, Afrin works great for me but I choose not to use it only under extreme circumstances because if the rebound effect. I have yet to try azelastine but I’ve researched it and like the reviews it gets. I’m hoping to get a prescription for it soon. That is next in my arsenal. I too had/have been suffering (agonizing)with sinus congestion mostly after weight training at night not as severely on rest days still enough to where I couldn’t breathe while I tried to sleep. It was horribly excruciating especially during my last cycle, the pressure was insane. I posted about my issue in another discussion. Post exercise rhinitis is what the consensus was. During physical exercise my nose would be clear as a bell but about 35-45 minutes after training I’d plug up with sinus pressure and blockage, worse when I’d lay down. I had originally thought that the pump supplements like Citrulline and beet root powder I was using were exacerbating the condition so I refrained from their use intra workout. That helped some on the pressure but not entirely. I couldn’t lay down to sleep as my sinuses would plug up severely, which consequently gave me anxiety and a rapid heart rate because I couldn’t breathe. For months I suffered from insomnia, sometimes getting a few precious hours of sleep a night(being a shift worker doesn’t help matters either). I had to sleep on my couch in an upright position, use a decongestant pill, and Flonase in each nostril, Vick’s menthol helped to open up my nasal passages or more accurately shrink my swollen turbinates so I could relax and sleep. Currently I use a nasal saline wash, once a day, an antihistamine such as Claritin D or loratidine, a decongestant such as Sudafed (because it’s allergy season in Texas) and Flonase 1-2 sprays in each nostril once a day. I just try to use the least amount for results. This works for about 24 hrs or so then I repeat. So far this has helped me get some rest and I’m finally laying down in my bed to sleep. Thanks again for this post you’re a life saver man!!

I’ll add my 2 cents (as I am probably more on the severe spectrum of sinus infections, even after sinus surgery and many different treatments I still suffer) and can provide addition items to consider with your doctor, all which I have done personally

1) Allergy shots / testing - ENTs are now recommending this as irritants cause nasal irritation, which can be a starting point to trigger sinusitis. This is more of a preventative measure. Some ENTs do this in their office, but these docs probably not receive as much training as as Allergy and Asthma specialist

2) Immune system testing (IGG levels) - they Test your immune system to make sure your body can properly fight infections. If not, this may be a cause of recurring sinus infections, and would have to get infusions

3) Microbiology testing - ENTs can swan inside the nose to get a culture to try and identify the pathogen, which they they can prescribe the correct antibiotic for optimal treatment. THIS IS MY SOAPBOX - BE CAREFUL WITH CIPROFLOXACIN OR ANY FLUROQUINOLONE ANTIBIOTIC. THESE HAVE BEEN KNOWN TO CAUSE TENDON WEAKENSS. (I popped my bicep tendon while on a round of Cipro for a sinus infection).

4) Antibiotic / steroid additive to sinus rinse - mainly prescribed through a compounding pharmacy, instead of taking an oral antibiotic that has to work its way through your system, more are prescribing antibiotics and steroids in powder or liquid form that are added to the nasal irrigation system for direct entry and application of the medication to the affected area. Only downside is that insurances don’t cover a lot through a compounding pharmacy.

5) Sinus infections can cause bloody nose (because the lining becomes inflamed, then people tend to blow their nose more while having a sinus infection which can irritate things even more). Afrin is a blood vessel constrictor so it will help if you have a bloody nose. Again afrin is not for everyday use but good to keep around if you have bloody noses frequently

6) Add Johnson’s baby shampoo to sinus rinse (just a very small amount) - Baby shampoo is a surfactant, meaning it helps prevent and wash away tension of a liquid (meaning biofilm can form in nasal cavaties thereby breeding bacteria, the wash helps prevent and remove these biofilms).

Again this is my own personal experience and does not substitute a visit to your ENT and the above does not provide medical advice. Medical advice should only be obtained from your healcare provider.

I had a septoplasty with turbinates shaven down in 06'

A couple of years later a different ent said the doc did a poor job and suggested another procedure.

Im always getting sinus infections and use a netipot and flonase fairly regularly. I have to sleep on my stomach or my snoring wakes my wife.

Just had a sleep study done and will be getting an oral appliance - hoping that helps. Ill also try the baby shampoo trick.

When i was suffering so badly and the oral mega dose levaquin wasnt working i was desperate and was looking into nasal abx. I couldnt find any great data on it but said eff it and dissolved a few crushed up levaquin in a saline nasal spray lol. Not sure it worked lol.


When i added the 4mg injectable dexamethasone to my nasal spray that worked extremely well within a day. Im lucky enough to have access to ct imaging and i look a quick axial slice a few days later showing my turbinates much smaller. that quick reduction in inflammation allowed the nasal rinses with baby shampoo to start working and in combination with oral levaquin.

Its so common for people to have turbinate issues. I also think, in my opinion, it may be even more common in AAS bodybuilders due to blood pressure issues and increase blood counts of like hemoglobin, mast cells, eosinophils etc.


Someone had also mentioned oral antihistamines. Good point. While benadryl works great at night i will occasionally take zrytec during the day if im gonna be around known allergans but truthfully i dont have to do it too often anymore just by using a low dose of nasal spray daily and daily sinus rinses.


Related but a bit off topic. I have an enlarged pulmonary artery. Its like 3.7cm. Decent size. May its incidental but ive had such difficulty breathing at night for years that id wake up hypoxic almost every night that i wonder if my sinus issues resulted in some pulmonary hypertension. Im finally sleeping fine, wake up well rested and not short of breathe.

Im convinced that certain aspects of AAS like BP increase, increased hemoglobin, mast cells and eosinophils from being on cycle can contribute to turbinate dysfunction. Just a theory.

I dont think what you were experiencing is post exercise rhinitis in the classic sense..more so the catecholamine increase during exercise was helping you breathe better...but once that stopped when you got home and HR settled down then the turbinates swelled up again. This happened to me all the time. In fact, id stay at the gym longer just so i could breath lol.

Glad you are breathing better. Its hard to enjoy life with your mouth open like a big mouth bass all the time.

Yes, as soon as I use Afrin I'm pretty good for 4-7 hours of good breathing. Then I have to use again.
Do you think I just have enlarged turbinates? I have barely experimented with allergy medication and stopped because I was not getting much, if any, relief.
But maybe I should try this flonase route.

Yes, as soon as I use Afrin I'm pretty good for 4-7 hours of good breathing. Then I have to use again.
Do you think I just have enlarged turbinates? I have barely experimented with allergy medication and stopped because I was not getting much, if any, relief.
But maybe I should try this flonase route.

Afrin works by shrinking the turbinates with some maybe getting into the paranasal sinuses. Doesnt do anything structurally. So if you are getting relief from that then if you were to reduce the size of your turbinates then you should get similar relief.

I also tried the diluting afrin and what i did was kept using afrin at night for a bit and used bronkaid during day for a bit. Then i started cutting down afrin with saline more and more.. during that time i was upping my flonose dose.

Unfortunately the flonose wasnt strong enough so i then switched to nasal dexamethasone and i was able to stop afrin within a few days. Once the turbinates went down the nasal flushes were more effective and helped with that cause of the inflammation.
 
I have an issue with nose being clogged up all the time. It changes from right to left nostril . It doesn’t run or produces mucus, its just clogged and its hard to breathe thru the nose.
Is this the issue with turbinates ?

positional changes are pretty classic for turbinate issues as they swell laying down or when they are the dependant side down.

I have this for years, and every time I ask dr, he says allergies and to take Claritin or Allegra , which don’t help much

Yeah. If I wake up at night I can spray the one nostril side that is closed off and lay down on my side. Lay with the closed off side facing up and the side that is open goes down against the bed. The closed off side opens up in a matter of minutes for me now, just using straight saline spray.

lol i used to be so bad with alternating sides laying down and trying to only use afrin in one side and then laying on that side down. changing sides during the nite.

when i was in san fran i was umm "actively dating" and i remember multiple times unexpected winding up back at some random girls place for the night. I had to find a 24 hour pharmacy to get afrin or id never be able to sleep in someone else bed without like 4 pillows, humidifier, breathe nose strip... it was insane.
