I've just recently encountered a problem that as of last night got extremely worse and could have caused me to completely fold under my last rep of heavy squats.

When a set (of squats) becomes really difficult and I strain to grind out the last few reps I have been experiencing a great deal of pain in the back of my head.

I have carried the bar high (on my traps) since I can remember so nothing has changed in that regard and my form is very strict but I figure I must be moving my head in some way to produce this pain.

Last night when I reached the top of my last rep the pain was so severe that I really don't know how I managed to rack the weight...I just kind of slumped forward hoping the rack would catch the weight.

Like I said, I'm sure I must be doing something wrong, as it can't be just the intensity of the exercise b/c I've never experienced this before on any other movement - only squats. Until I can pinpoint exactly what I'm doing I would appreciate any and all suggestions.

I would say lighten the weight, focus on (ie don't hold your breath) your breathing, monitor your blood pressure.

Additionally, heavy weights can nudge a vertebrae in the spine out of place which can mess with blood flow in certain areas. Id consider getting an evaluation and adjustment from a chiro. Preferably one that is certified in ART.

nice points. i set the bar lower on my traps, not so much on my neck. theres a little groove right below your neck where....4 me....the bar rests perfectly. you might also find it more comfortable to get a little wrap around pad, i odnt like them but some find it more comfortable. also you could try a front squat. my .02

If your form was strict, you would have been carrying the bar properly.
Low on the traps in that groove between the traps and the rear delts.
get your blood pressure checked too. High blood pressure can cause what I think it is you are describing.
If you are on gear, your BP is probably up depending on the chemical.
 
I agree with most here.I think you do have the bar a little too high. Work on a lower bar position and you should be fine. Right in that groove as others have said. But also breathing is so important when into the last few reps of your set. it will cause you BP to skyrocket, esp as you come out of the bottom!!
 
I prefer to do squats with the bar higher up, as I feel it hits my quads better, but if that is hurting your head, definitely try lowering the bar. If it still persists, maybe consider doing some deep squats on the smith.
 
I know my BP is perfect, as I had a physical a short while ago and I'm not currently on so I'll definitely change where I carry the weight and concentrate on my breathing. Thanks to everyone for your suggestions!
