Need a good gym sign in sheet sample

I own a gym and to date I have not been making members sign in at the front desk as they come in. It hasn been a big deal but as it is growing I think it is something I want to start. Everyone signs a waiver when they join but i would like a additional one on the sign in sheet. I have looked everywhere online and cant find any good wording, does anyone have one with some nice verbage that they could copy down and let me use?

Lifting weights and using gym equipment can be a dangerous activity. Even many professionals are injured. Doctors fee's for injuries are extremely expensive in this day and age. As a gym member, you are not covered by any kind of health insurance from the gym. We ask that all gym members accept full responsibility and accept the risk for their own safety.

I understand that "insert the name of your gym" will not be liable for any injuries or illness that I might suffer. I hereby expressly waive any claim for compensation or liability arising out of any injury or illness that I sustain in connection with "gym name" and hereby release "gym name" its employees and all other gym members from any claim for personal injury or illness that I sustain, regardless of whether caused in whole or in part by the negligence or fault of Releasees.
