Insulin alternative
What about Prandin or Starlix. They are both similar to a fast acting glyburide/glipizide designed to be given before meals TID. Here is some information:
REPAGLINIDE(PRANDIN)
0.0 OVERVIEW
A. Repaglinide is an oral agent of the meglitinide class used for treating noninsulin dependent diabetes mellitus.
B. DOSING INFORMATION: The recommended dose ranges from 0.5 to 4 mg administered before meals; the maximum total daily dose should not exceed 16 mg.
C. PHARMACOKINETICS: Rapidly absorbed following oral administration. Extensively metabolized in the liver and excreted predominantly via the feces; no active metabolites. Elimination half-life: less than one hour.
D. CAUTIONS: Hypoglycemia is the major adverse effect; nausea, diarrhea, constipation, vomiting, and dyspepsia were reported at incidences similar to placebo and sulfonylureas.
E. CLINICAL APPLICATIONS: In the United States, repaglinide is approved for treating non-insulin-dependent diabetes mellitus as monotherapy or in combination with metformin or thiazolidinedione (pioglitazone or rosiglitazone). Repaglinide has significantly reduced postprandial blood glucose in type 2 diabetes; minimal effects on fasting blood glucose were observed.
2.1.1 ONSET
A. INITIAL RESPONSE:
1. NON-INSULIN DEPENDENT DIABETES MELLITUS, ORAL: 30 minutes (Wang, 1998).
B. PEAK RESPONSE:
1. NON-INSULIN-DEPENDENT DIABETES MELLITUS, ORAL: 60 to 90 minutes (Wang, 1998; Robling et al, 1992).
a. Single-dose: peak decrease in postprandial blood glucose (given 15 minutes prior to meals) (Robling et al, 1992).
2.1.2 DURATION
A. SINGLE DOSE:
1. NON-INSULIN-DEPENDENT DIABETES MELLITUS, ORAL: less than 4 hours (Wang, 1998; Robling et al, 1992).
I know these are not exactly like insulin, they stimulate your body to make its own insulin. Just a thought as an alternative to using insulin. Others may have used them or have opinions regarding using these agents. I personally have used Prandin with similar results to glyburide(some fat/some muscle). Caloric intake may have been to high.
If you need more information, let me know.
