With transdermals you get aggregation of the AAS in the stratum corneum (within the skin) which, for example with scrotal application, can lead to increased DHT (with test) in the prostate due to local 5α-reductase activity in the tissues in the area.

With intramuscular injections of unesterified AAS, you still get the classical absorption/distribution/metabolization/excretion as with esterified compounds, i.e., dissolution into the blood of the circulatory system where the androgen can do its thing systemically.
