Shoulder problems with lat work

I have an ongoing on and off problem with my shoulder that keeps popping up and is keeping me from lifting right now.

Any downward movement with resistence causes major pain in my shoulder between the front and middle delt. Also pulling my arm into the side lat with no resestence like laying on it or just pulling it in hurt there too. Presses with the elbow out are fine as long as the resistence is away from me i.e. shoulder and chest.

Does anyone have any ideas on this? Are there ways to work around it?
Everytime it goes away it always finds a way to come back.
 
Have you tried a narrow underhand grip on your pull-downs? Elbows more to the front? Pull at the elbow, not with the bi's. I love those and really feel them in my lats
 
Yes, it is anything with downward movements or if I put my arm behind my back. I really hope I didn't tear my cuff because also I can't do any dumbell pressing because rotating them out from my legs hurts but barbells with no rotation out are fine.
