Any one on Effexor/vanlafaxin ?

So ive been on Venlafaxin for several years now and i have alot of cronic pain so on morphine or morphine like meds also, now the doc vants me to try LDN aka Low Dose Naltraxtone for the pain, but i was reading up on Vanlafaxin and since it look alot like Tramadol it also binds to two of the opiod receptors and actually works as a partial pain med, using LDN wont this fuck with the effect of Venlafaxin since it cant bind to the receptor then ?

Well ive been on it for years for depression, Suicide thougths and as a Moodstabiliser, but for some reason this last year it started to loose its effectiveness, so we increased dose from 225mg ed first to 262,5mg and later to 300mg and no response at all from my body/mind, so something has changed. Before ppl start to recommend diff kinds of therapy etc ive been thru it all, ive been on ssri/snri meds for over 20 years, my psycological shit started at a very early age so yeah ive tried it all and do have different methods to cope usually :) and i have a therapist that helps me. So at the same time i have a lot of different issues with pain from all kinds of accidents, car, job, beatings etc so yeah alot is going on :D

Depression can cause some nasty aches and pains. Maybe it’s tied into some pathway for that. Which pathway, I have no idea. 🤷‍♀️

Oh absolutely, having had serious chronic pain since childhod i know pain better then most and the brain itself is deff a big player here.

I would ask the pharmacist. He is suppose to be expert on interactions.

You have to get the dosage down on the opiates for sure! Even LDN will kick the opiate off of the receptor. I used it years ago...I made a thread about it too. I felt like shit pretty quick because the pain doc didn't have me taper.

Oh yeah offcourse, but LDN is more used off label as a pain med since it hasent really been greenligthed for it.

The pain controlling properties are mostly because of alpha 2 activation via noradrenaline and indirect stimulation of serotonin 1a autoreceptors.

The a2 activation requires the stimulation of Mu opioid receptors (MOP)

In mice, naloxone inhibited the a2 activation of VEnlfaxine by blocking MOP. NALTREXONE is a stronger antagonist of MOP than naloxone.


So in theory, if you were a rat, yes. It will fuck up the pain relieve properties.


But all of this has 1,734 different other variables.

Also, real life effects are rarely = what we would assume will happen looking at the molecular level.

Trust your doc for a month. If it doesnt work, fire him.

If you’re in pain, naltrexone fucjing up Effexor is your least worry. Especially if you need opioids for pain control
 
Also I have no idea the human equivalent doses in the studies and also…..we can only extrapolate so much across species.

A little better than a wild guess though
 
Man,! I'm truly sorry you are going and have been dealing with such a difficult thing, i should be more grateful about my life and try not to complain too much, it's true there's always someone going through more sht than you,. Best wishes and never give up!
 
And I don’t think Effexor directly agonizes MOPs; they just play a role in the effects of the a2 receptors in the locus coeruleus
