Dutasteride 0.5 mg twice per week = Finasteride 5 mg daily w/ Less Side effects

For those asking about dutasteride or those saying they can't use finasteride even at low dosages; Dutasteride at a dosage of 0.5 mg twice per week will generally lower DHT by 70%, the same as 5 mg finasteride. In fact, it may be better since finasteride has a spread of 50-90% DHT reduction depending on the individual whereas dutasteride is more consistent at lowering DHT.

The studies also show a slight advantage in hair regrowth from 0.5 mg dutasteride x 2 per week versus 5 mg finasteride daily.

Anecdotally, MANY patients who could not touch finasteride even at low doses have no side effects from dutasteride or only very mild ones. (slight libido reduction, slight brain fog). Often resolving with time on their own.
(https://pubmed.ncbi.nlm.nih.gov/15363566/ " Onset of new drug-related adverse events were reported most frequently at the start of therapy and declined over time in patients receiving dutasteride.")

I will add that Finasteride has one effect that dutasteride does not that may be the reason side effects can be severe for some on finasteride but not on dutasteride. Finasteride inhibits an enzyme called PEMT. This enzyme is responsible for converting noradrenaline into adrenaline and also for producing choline. Both noradrenaline and choline can effect mood and cognition. (Anti-depressants like wellbutrin can increase/affect noradrenaline and anti-alzheimers drugs increase choline or acetylcholine effects in the body).

Another thing i will add is that as we get older, BPH/prostate growth can become a real problem, especially for men using AAS and HGH. This decreases quality of life and recovery as you wake up multiple times during the night to urinate. A 5-alpha reductase inhibitor can still shrink the prostate even on high doses of AAS/testosterone as DHT is the main driver of this growth/swelling effect, as always, prevention is the BEST medicine there is.

Here is an experienced hair loss doctor from Spain discussing his experience with low dose dutasteride (0.5 mg twice per week) in his patients who could not handle finasteride:

That has been my protocol since I started(.5x2). I get zero side effects

I was using .5 MG of fina per day but recently switched to .5 MG of dutasteride every day along with 10mg minoxidil per day. I am having no side effects at this dosage. I started fin/minoxidil last August, and switched to duta/minox a couple weeks ago. It seems like my hair loss has stopped. If I run my fingers through my hair, none falls out these days. I'm hoping the duta/minoxidil combo will cause some regrowth in the months ahead.

First of black beard thank you for all the threads you have started here on hair loss. You are an encyclopedia of knowledge and experience on the topic.
I was on avodart for years off label us brand name script as well as lovaza before the insurance carrier called. They wanted documented evidence of bph from a urologist. The lovaza and avodart were both free but they said I had no medical need low triglycerides taking lovaza and no bph for avodart.
GSK never pursued phase 3 clinical trials for hair loss back in the early 2000s I was on a hair growth forum and everyone was up in arms since avodart lacked the sides finasteride had. Their reason they only estimated it as a 500 million dollar sale drug and they were only into billion dollar products like valtrex and zantac etc.

What are your thoughts on stinging nettle shampoo? Sorry if I missed it in one of your threads. I have been using stinging nettle shampoo and my hair has never been thicker. My hair line has receded a bit at 55 but that nice little spot at the top of thechead became noticeable. My wife likes to kiss the top of my head and say I can't wait until you have a cue ball! LOL. I tell her one bald head is enough.
Thanks again for the threads

The shampoo can't hurt. I generally recommend nizoral/ketoconazole shampoo twice per week, the stinging nettle is a better milder option for the other days.

Ketoconazole shampoo can improve the response to finasteride/dutasteride significantly in many people, best results are usually seen after 6-12 months of CONSISTENT use. (2-3 times per week, too often can make hair frizzy/dry).

It sounds like you are still in an early enough stage to reverse the hair loss. Dutasteride 0.5 mg 2-3 times per week, ketoconazole shampoo 2-3 times per week and 2.5 mg oral minoxidil, you have a good chance of filling in the bald spot completely and recovering some hairline (the part of the hair line that is thin/sparse, slick bald spots are much less likely to wake back up). Most importantly you would stop the hair loss from progressing.

You can source duta/keto/minox from the usual online indian pharmacies like reliableRx or alldaychemist, most primary doctors will Rx them fairly easily as well and a third option are online clinics like Roman or happyhead, a lot of times they will fill your script with just an online chat with a doctor (no video or phone call).

Oral minox ED at 2.5mg? Any sides with that?

Also, does anyone know any sources for Europe?

Thanks!

Great thread BB, thanks for sharing.

The Keto shampoo (Nizoral 2%) is very drying on the scalp, used to use another formulation called Regenepure but they no longer use keto in that; BB, do you have any suggestions for nicer shampoo formulations containing Keto?

Also, for those taking Ivabradine and ketoconazole, there is some interaction I believe (Keto increases absorption? Something like that).

Oral minox ED at 2.5mg? Any sides with that?

generally, no. Some people get some mild dizziness or heart rate increase that passes after 1-2 weeks. Sides are very rare and it is extremely effective for regrowth.

All day chemist and reliableRx ship to Europe last i checked.

Yeah, limit to x 2 per week. Apply as soon as you get into shower, you want minimum 5 mins contact time. Rinse off once you're done with your shower. (I like to brush teeth/shave while the nizoral is working)

Use a good conditioner intended for dry scalp after 5 mins of nizoral right on top of the nizoral. (I use head & shoulders 2-in-1 almond oil shampoo conditioner and have no issues with dryness or itching which i did before using h&s and nizoral more than twice per week).

Thank you BB for all the info. Should you recommend consecutive days like the doctor in the video or is Mon - Wed - Fri just as good (or Mon - Thu if taking two per week) or does this really matter?

It should not matter.

Not only due to the half life of dutasteride, but also because 5-alpha reductase takes a while to be remade after its disabled by fina/duta. Also, fina/duta bind to scalp/follicle/prostate tissues for even longer than their half life will tell you.

Do primary care doctors prescribe Ketoconazole shampoo? Or what kind of doctor would you go through for this?

Is 0.5mg every day doable ?

Just like with finasteride, the higher the total weekly dosage, the higher your risk of side effects.

0.5 x 2-3 times per week is the sweet spot for most men (around 70% serum DHT reduction). Daily can increase the efficacy also the risk of side effects. Many men take it daily with no sides, but many men will experience sides on a daily dosage.

Adding nizoral and/or minoxidil will get you more bang for your back without increasing the risk of sides.

Another great informative post black beard , one question , would using biotin along with fina and oral minoxidil have any extra benefit? I know biotin is definitely the weakest link in the chain , they sell pretty high dosed biotin , 5mg and even 10mg , would that help in your opinion and if so would you go with 5mg or 10mg daily of biotin ? Also would you recommend durasteride over finasteride ? Thanks again for great information

What would be some of the sides of taking it everyday ?

It's about that taking daily will give you sides, only increase the risk or intensity of sides. Many men take finasteride/dutasteride with no side effects or only mild ones.

Everyone has a different sensitivity to finasteride and dutasteride. Some guys will be zombies on 1mg finasteride and others can take 5 mg daily with zero side effects. Same concept applies to dutasteride.

It doesn't hurt, if you're not using a good multivitamin that contains biotin already, taking 10 mg could possible help a bit. Lots of anecdotes that it somewhat helps. Optimizing vitamin D, ferritin (iron), and getting enough protein (or adding collagen) are some easy, low hanging fruit for someone looking to get a few more % improvement once you address the main cause (DHT).

I generally lean to recommending low dose dutasteride (0.5 mg 2-3 times per week), actually less likely to cause side effects in my experience. Finasteride can possibly affect choline levels and noradrenaline/adrenaline levels, these are important neurotransmitters and precuros involved in mood, arousal, sleep and other body processes which is possibly one reason finasteride is more likely to cause sides.
Glad to help, always

@SOUR DIESEL Thank you for the kind words brother, this is my absolute favorite community. I always love a good discussion too. The feeling is mutual.
