What happened is he had a surgeon that only "Thinned out" the glands, he did not remove them. And this is common, the reason being if you remove the entire gland, and the patient has a thicker layer of fat, or the potential for having fatty tissue around the area you will be left with pocket, or divot where the gland was, not something you want.
When I had mine done the Doc took out as much of the gland as he could find, THEN thinned out the fat in the surrounding area, but I was lucky because I didn't have much potential to be fat in the lower chest area, then he grafted fat back into the pocket left from the gland removal.
I wouldn't call the problem that your friend has the result of a botched surgery (It's common), but the result of a surgeon not looking into the future (Meaning your friends continued hormone usage I assume), or maybe doing a "Textbook" job, that doesn't include the steps my Doc took.
Also my Doc installed "Drain tubes" that came out of my chest that remained during the first week of recovery. And I am drawing an inference here but the other two people who I know (And seen before/after) that had the same procedure but didn't have drains, it took longer for their chest to "Look" recovered, and the nips had a remaining puffy look (Stretched/pointy), where as mine healed small, flat and tight to the muscle.
