For L-carnintine to work, it has to be injected. Oral simply does not work.

N-Acetyl L-Carnitine (ALCAR) is a modified (acetylated) form of L-Carnitine, a nitrogen-containing compound derived from L-Lysine. The carnitine molecule is necessary for the proper movement of fat molecules into the mitochondria for oxidation (fat burning). It seems to be beneficial for individuals suffering from a large array of maladies, including anorexia, chronic fatigue, coronary vascular disease, diphtheria, hypoglycemia, male infertility, muscular myopathies,, Rett syndrome, as well as preterm infants, dialysis patients, and HIV+ individuals7 ALCAR also seems to be very useful for age-associated cognitive impairment, such as Alzheimer’s8.
A controversial supplement for many years, its effect on exercise performance has not been established3. Similarly, studies examining the effect of ALCAR on fat oxidation have been mixed, with some studies showing a beneficial, lipolytic effect12, and others showing no effect4, 5, or even a shift toward greater carbohydrate (less fat) oxidation1. Animal studies suggest a nitrogen sparing effect of L-Carnitine during fasting6, but a study of humans undergoing a calorically restrictive diet and exercise program found no effect11. However, individuals with impaired muscle performance (possibly suggestive of deficiency) may benefit from L-Carnitine supplementation. A study by Pistone et al. 9 found that older individuals with highly fatigable muscles increased muscle mass, lost body fat, improved muscular performance and their blood lipid profiles simply by supplementing with L-Carnitine.
Supplement Use
L-Carnitine is more bioavailable when consumed in the normal diet, although supplemental forms of L-Carnitine are also absorbed (up to 6 g dose,14-18% absorption). High doses can cause gastric distress11. N-Acetyl L-Carnitine is absorbed intact, has similar pharmacokinetics as L-Carnitine, and elevates blood levels nearly 50%10. ALCAR may be especially useful for older individuals suffering from muscle fatigue, cardiovascular disease risk and cognitive impairment8. ALCAR could also be taken with alpha lipoic acid to prevent mitochondrial damage, which occurs as we age 2.
