Total testosterone of 290 at 21 years old. Options?

Story: Back in 2013 I had a blood test done which showed total testosterone at a level of 410 and because I was 19 at the time I trusted the doctor when he said it was normal. In January 2015 I had another blood test which showed a level of 422. I was looking up testosterone levels on the internet and realized a level of 422 is low for a 21 year old male so I wanted to get another test to make sure. Just a few days ago I tested at a level of 290 on a third test.

Feels: To be honest my libido kind of sucks and it’s sucked since high school. I can still force myself to get a hard on that does get very hard but I’m unable to get one without stimulation. Also if I don’t force myself to get a hard on I won’t have one period (for weeks at a time). My morning wood isn’t great either because it’s more like a semi hard on at 50% and it’s been that way for years. I still find women very pretty but I don’t really have any sexual thoughts. During the day I feel tired and fatigued while at night I have trouble falling asleep as well as waking up too early. I still get around 7 to 8 hours of sleep a night but feel tired in the morning.

Concerns: I’m just wondering what my options are at this point. I’ve read a number of threads in this subforum and I realize just how difficult it can be to get a prescription for testosterone injections. My level is also above the bottom end of the reference range which is 250. My other concern is when I go see the doctor he will want to do another blood test and if that blood test shows up higher than 300 he might say I’m “normal” which is completely wrong for a 21 year old. Or he might even say a level of 290 is normal because it’s above 250….. I’m also wondering if I find a doctor who will treat it if he’ll just prescribe the gel so I get my level into the 400s again. Since I have a low level and it needs to be treated I’d rather have my level be at the upper limits of normal with injections.

Anyways sorry this was so long but I’m just wondering what exactly are my options and what is your advice?

I'm sorry for the double post but I forgot to add that I've never done AAS before so I'm not sure why my level is so low. :/

I would say at your age HRT should be a last resort. You are wrong and pinning for years would suck.

What time of day were the blood tests?

What is your weight, height and estimated bodyfat if you guessed?

Do you know your other hormone levels - thyroid, estrogen?

I would get a full blood work and assessment on life, diet, stress etc. Then you can try to optimise and potentially even use clomid to help increase etc.

Thank you very much for your reply. I'm 170 lbs at around 15% body fat (I know it's not great but I still have a V-cut and visible veins on my calves). The blood tests were taken between 9 am and 11:30 am. On a previous test my estrogen was fine, not too low or too high. I workout every week and walk at least five miles a day. I'm not stressed out, I take DAA, zinc, magnesium and a multivitamin. I eat enough to gain weight but I'm still getting weaker in the gym and it takes me around a week to recover. I just think my high "normal" level of testosterone is in the 400s due to the two tests in 2013 and 2015. Currently it's at the 290 so the prospect of living like this isn't very comforting. I will get further blood tests once I see my primary care doctor in a few weeks. At this point I only want to feel how a normal 21 year old is supposed to feel.

21 and low test? Doesn't sound healthy.. If you have abused AAS before without proper medical guidance but the guidance of ignorant fools at the gym or some internet bs then sadly that's plain ignorance and stupidity. Which many of us commit at early stages of our life, seek medical professional, visit a specialist, get a full physical and anotate your prior history with AAS and tell the professional of your prior AAS usaged... Once you are honest w yourself then things could get better for you...

Sorry for the late reply but I just had an MRI yesterday because my doctor thinks I have some kind of tumor in my pituitary that's causing the low levels. It doesn't make much sense because I tried HCG from an overseas pharmacy and my testosterone level didn't change at all (still in the 300 to 400 range). But my FSH and LH did drop to significantly under the bottom end of the reference ranges for those two tests so I know the HCG is real HCG since it was normal before. I told the doctor that and he still wanted me to do the MRI so I did it.

Alright looks like I am correct. As I said before my testosterone level was 400 ng/dL while taking the HCG I bought from India (my level before taking the HCG was also 400) and I went off of the HCG for a week then got my testosterone level retested (along with getting an MRI done) and my testosterone level came back at 136 ng/dL so I can say with 100% certainty that the HCG I bought works. This also confirms my view that I have primary hypogonadism (wel or at least a low maximum testosterone level that I can achieve through any means) because direct stimulation of my testes via HCG failed to elevate my level to a normal level for a healthy in shape 21 year old male (which is around 1,000 ng/dL). But of course my level is just not severe "enough" to treat according to doctors (because apparently having 0 erections in the morning and 0 erections during the day is NOT severe according to doctors.... sigh)). Finally the MRI showed a 3mm cyst on my pituitary but my doctor said people can just be born with them but he wants me to get another MRI in a few months to see if it has enlarged or remained the same size. This cyst cannot be the cause of my problems because even if the cyst reduces my production of LH (theoretically because based on tests it does not) HCG acts like LH in the body by directly stimulating the testes to produce more testosterone which bypasses the pituitary's regulation mechanism.

So it looks like my only option at this point in time is to figure out how I can convince a doctor to give me testosterone replacement therapy.

Ii agree see ans endocrinologist. They will put you on a regimen of clomiphene citrate (clomid) if that raises your testosterone they will put you on HCG.

I tried HCG that I bought and my level stayed the same at the low 400s. I know the HCG is real because a week after I went off the HCG and got my level retested it was 130 something. I'm going to see an endocrinologist and ask him for injections because I think that's the only thing that will work at this point.

could be genetic who knows. Remember back in the 50's and such testosterone tests werent around were they? so how do doctors know what normal is, when its quite rare to see natural higher levels , most guys I see are in the 300-600 range. everyone levels are naturally different.

a few questions , are you cutting?

are you keeping fats in the range of 20-30 % (or higher if you want, I prefer not too high fat myself)

any recreational drugs that have caused this? (youd be surprised what recreational drugs can do to htpa system)

My dad got his a couple years ago at 230 something, a year later at 300 something, and this week came back at 460 , hes 61 btw and he took his test in afternoon after lunch. I am thinking the whole fasting thing is just unnecessary and bs and its best after a meal, esp with fats.

Hey brother. When I was 21 my testosterone levels were 188ng/dl. I felt just as you described it was terrible. I was also worried that I would not be able to discover a doctor that would prescribe me Trt. Online Clinics are your best bet. I was RXed 210mg every 5th day Hcg, and anastrozole. After a few weeks I felt amazing! There are clinics out there that will help you but they do not take insurance. But they are more "qualified" i would say than your local endo that is likely to put you on something like 50mg test a week or 100 mg every 2 weeks, which will lead to a roller coaster effect. There are plenty of clinics out there and they are not as expensive as you might be thinking.
