Total testosterone of 290 at 21 years old. Options?

could be genetic who knows. Remember back in the 50's and such testosterone tests werent around were they? so how do doctors know what normal is, when its quite rare to see natural higher levels , most guys I see are in the 300-600 range. everyone levels are naturally different.

a few questions , are you cutting?

are you keeping fats in the range of 20-30 % (or higher if you want, I prefer not too high fat myself)

any recreational drugs that have caused this? (youd be surprised what recreational drugs can do to htpa system)

My dad got his a couple years ago at 230 something, a year later at 300 something, and this week came back at 460 , hes 61 btw and he took his test in afternoon after lunch. I am thinking the whole fasting thing is just unnecessary and bs and its best after a meal, esp with fats.
Not cutting, yes I eat enough fats and haven't used drugs ever. I don't even drink any alcohol. My dad has a 410 level at the age of 55 haha so his is higher / the same as mine. Thanks man

Hey brother. When I was 21 my testosterone levels were 188ng/dl. I felt just as you described it was terrible. I was also worried that I would not be able to discover a doctor that would prescribe me Trt. Online Clinics are your best bet. I was RXed 210mg every 5th day Hcg, and anastrozole. After a few weeks I felt amazing! There are clinics out there that will help you but they do not take insurance. But they are more "qualified" i would say than your local endo that is likely to put you on something like 50mg test a week or 100 mg every 2 weeks, which will lead to a roller coaster effect. There are plenty of clinics out there and they are not as expensive as you might be thinking.
Thanks man I'll probably give that a shot because the doctors I've seen have been terrible. This one guy told me to take some herbal supplement he was selling in his office for $200 and the same herbs were $30 on amazon. Not trying to say herbs would do something. I actually told this guy I very rarely (~1%) have morning erections and the best he could do was tell me to take a herbal supplement. I asked him if he could actually help and he said no so I noped out of there.

A close friend of mine has a son is your age, and he has a similar testosterone level, with a very high prolactin level. He has normal LH and FSH, in fact, all of his hormones besides testosterone and prolactin were in the middle of the normal range.

He saw an endocrinologist who insisted he get an MRI to check for a tumor on the anterior pituitary (where LH and FSH are created and released). The MRI came back and revealed a 1-2mm growth. The doctor did not seem surprised and said that these type of tumors are extremely common.

I would definitely see an endocrinologist. My friends son was told that the tumor was benign and wouldn't kill him or anything, but he is going to get yearly MRIs and blood work every 3 months to monitor levels. He is now doing TRT at 150mg/week and feels better than he has in his whole life.

I should mention, my friend is 68 years old and has a testosterone level of 510...

EDIT: Just saw you did get an MRI and they found a growth. It isnt the end of the world. Sounds like a very similar situation to my friends son.

HCG and Clomid are not used by endocrinologists for TRT these days, and they have side effects of their own. Do not worry about fertility. They can give you drugs that will help you be plenty fertile, or you can freeze some sperm now.

It should be noted that when on TRT your sperm count will be lower, but it will still be enough to have a kid... My buddy has gotten his wife pregnant two times and both of the kids are very healthy. I dont think he has ever dropped below 500mg/week for the past 10 years!

Thanks man. I actually have a prolactin level that's towards the lowish end of normal so it's not high or anything but yea. The growth is just this tiny cyst that up to 25% of all people have. Your friend's level is pretty good for being 68 because even my dad has a 410 in his mid 50s and I have a 400 at best. Then yea I'll be getting another MRI done in around 6 months to make sure the size of the cyst hasn't change. But if it does get too big it's not really that big of a deal because it'll just have to be drained.

Unfortunately for me I'm in the "low enough to suffer but too "high" to treat" group. At best I can get an overpriced $250 prescription for clomid or $500 a month HCG. I asked my primary care about trying HCG and the guy said lets just wait a few months and see first so that's why just I got it elsewhere. Told him this after and he didn't seem to care. So far no one has even hinted at the possibility or even considered TRT. Do you think doctors are just afraid or something?

No, I dont think you are talking to the right doctor. The guy I knew had the doc ask him if he would consider HRT. He said yes and the doctor was relieved. He said it was the most direct approach to treating his condition, with the least side effects.

Thanks. I don't know man, even an online clinic said no because I tested at 358 for them and I needed to be under 350 to qualify. Kind of bullshit but whatever. I'm sure a good doctor is out there but wait lists are just way too long to see someone then I always take the chance they'll say no again. I've been trying since the start of April this year (2015) and I'm not any closer now vs 6 months ago. I'm in my senior year of college and I just want to have a good time before I graduate but it's kind of hard with such a low T level as you can imagine due to the symptoms.

You should just get some gear, self medicate the TRT, get lab tests done once every few months to make sure your numbers are all good, and then when you have some down time get off the gear and have your test level checked, the number will be very low.

Thanks man, I'll probably end up doing it myself because I've read a lot of doctors want to do the testosterone gel instead of the injections because they're under the misguided belief the gel is better. Then they might require me to go into the office to get a shot every 2 weeks or only give me enough to get a 600 level. Kind of like the HCG I guess. It was just easier and cheaper to get it myself. I know my primary care doctor has no problem ordering blood tests because I've had 6+ in the last year with him. I'll be sure to make sure everything stays ok. I'll also make sure to donate blood if needed. Thanks for your help bro.

Story: Back in 2013 I had a blood test done which showed total testosterone at a level of 410 and because I was 19 at the time I trusted the doctor when he said it was normal. In January 2015 I had another blood test which showed a level of 422. I was looking up testosterone levels on the internet and realized a level of 422 is low for a 21 year old male so I wanted to get another test to make sure. Just a few days ago I tested at a level of 290 on a third test.

Feels: To be honest my libido kind of sucks and it’s sucked since high school. I can still force myself to get a hard on that does get very hard but I’m unable to get one without stimulation. Also if I don’t force myself to get a hard on I won’t have one period (for weeks at a time). My morning wood isn’t great either because it’s more like a semi hard on at 50% and it’s been that way for years. I still find women very pretty but I don’t really have any sexual thoughts. During the day I feel tired and fatigued while at night I have trouble falling asleep as well as waking up too early. I still get around 7 to 8 hours of sleep a night but feel tired in the morning.

Concerns: I’m just wondering what my options are at this point. I’ve read a number of threads in this subforum and I realize just how difficult it can be to get a prescription for testosterone injections. My level is also above the bottom end of the reference range which is 250. My other concern is when I go see the doctor he will want to do another blood test and if that blood test shows up higher than 300 he might say I’m “normal” which is completely wrong for a 21 year old. Or he might even say a level of 290 is normal because it’s above 250….. I’m also wondering if I find a doctor who will treat it if he’ll just prescribe the gel so I get my level into the 400s again. Since I have a low level and it needs to be treated I’d rather have my level be at the upper limits of normal with injections.

Anyways sorry this was so long but I’m just wondering what exactly are my options and what is your advice?

If i were you I would'nt go on trt unless you have major issues with libido and carrying muscle. sex drive can be affected by your current mind state and standing in life. trt is not all that it is cracked up to be. Pinning sucks. If you use too much trt you will run into androgenic sides down the road and this adds up over a lifetime.

Yea like I said I don't have any morning erections. I'd pretty much do anything to at least have those again because it kind of sucks waking up with a limp dick. Libido also sucks and whenever I ejaculate it takes a 12+ hours before I'm even physically able to get an erection again (let alone the 0 sex drive for a couple days which is lower than the usual 1 to 2 on a 10 point scale). My prolactin is on the low end of normal too. Yea I could use too much but I think if I maintain with a dose to keep me in the 1,000 to 1,200 range which is the top of the reference range then get my blood monitored every few months I'd be pretty safe right?

That is a pretty high testosterone level. I am sure youd be fine, but make sure to get regular blood work just to be sure :)

Yea having a max of 400 ng/dL at 21 and going down from there would suck so if I don't start now for sure I'll need to start before 30.

I just figure that if I'm going to need to inject for life I might as well maintain a level that's at the upper end of the reference range which is 1,000 to 1,200 for the places I've had my blood tested at. If doctors think 400 which is 50 above the 350 low end is "fine" then 50 under the high end (1,150) at 1,200 is fine too :p I'll make sure to get blood tests done and I'll stay at that level assuming nothing bad happens.

The risk of serious medical conditions can go up with having elevated testosterone levels for an extended period of time. Most people are fine. I just wanted to make sure you understood that your testosterone level being in the upper range isnt what you need to have tested... in fact, I would not even test it because then its on record that your levels were there. I would just check things like hematocrit, LDL/HDL, and cholesterol.

And I'd need to test stuff like E2 to make sure that stays ok. Yea when I said tests to make sure I don't have problems I meant making sure all my labs stay in the normal range.

Found that staying on a high hrt dose too long can ruin your hair, skin, blood pressure, and f with your mind. I dont believe everyone needs to be at 1200 test level all the time.

Test doesn't make you look that great when compared to the results other aas can give you anyway .

hrt is probably only going to give you a slight advantage in the gym and libido over your natural self. and like i said alot of times the libido has to do with your mental state and no amount of test can fix that if thats the case. for me hrt at 125mg is noticeably too androgenic to be on for life.

This is good first hand information. Take what he is saying to heard.

A doctor that works with us recommended 100mg tren E/week for TRT... he said he had great results with it. Keep in mind that those crazy high test levels are there on the range for the same reason as the crazy low levels are.

800ng/dl would be a solid testosterone level.

Thanks man, I'll see how it goes and I'll watch out for a bad reaction if I get it. I'll aim for the highest level I can do with out any negative side effects and shit. I just started the injections a few hours ago so hopefully everything goes ok.

Will do. Yea it could be too high. Only way too see is to do it and see if there are any negative sides. I don't really want any sides so I'll maintain the highest level I can without the sides.

The highest level of testosterone must be just exactly in your age (18 – 21 years) I recommend you to visit good specialist ASAP. Good luck bro

Thank you very much for your reply. I'm 170 lbs at around 15% body fat (I know it's not great but I still have a V-cut and visible veins on my calves). The blood tests were taken between 9 am and 11:30 am. On a previous test my estrogen was fine, not too low or too high. I workout every week and walk at least five miles a day. I'm not stressed out, I take DAA, zinc, magnesium and a multivitamin. I eat enough to gain weight but I'm still getting weaker in the gym and it takes me around a week to recover. I just think my high "normal" level of testosterone is in the 400s due to the two tests in 2013 and 2015. Currently it's at the 290 so the prospect of living like this isn't very comforting. I will get further blood tests once I see my primary care doctor in a few weeks. At this point I only want to feel how a normal 21 year old is supposed to feel.
You are very good candidate for HRT/TRT at 290!!! Have you been denied?
If so, why not go the Arnold Schwarzenegger route? Cut down your walks to 3 miles 3x a week and exercise 3x a week and take the weekends as your resting/off days.
