Actually very accurate. THey are NOT admissible in a court of law, period. THey are used in many arenas for vetting of employees, cprivate contractors, etc.. but not in a court of law.

individual judges have discretion to decide if a polygraph will be admitted based on certain criteria.In some cases, like in military courts, the use of polygraphs has been banned outright. But generally speaking, it's an individual judge's decision. Each judge must make an evaluation based on the information provided, and so each court can decide differently.

I think the tests for the most part are very accurate. The only times I have seen the tests not work is when the subject is under the influence of certain drugs. The test giver seems to be able to tell that the subject is under the influence though and so doesn't administer the test.

It is not 100% accurate, but very few things are. If you look at the results of those tests and then the history of the female and the particulars of the situation where the assault is alleged to occur, I would argue that the allegation appears to have a good chance of being false.
