Why do some people do cable internal/external rotations this way?

What's up fellas? I was just wondering what is the benefit of doing cable internal/external rotations with a towel place under your arm pit? Like you can see here:

What is the difference between doing it with or without a towel roll? My good friend muscle96ss gave me his expert opinion on this, I was just wondering what the rest of you guys thought.

And is there a place online I can order a pre made towel roll that was made for this purpose. Perhaps maybe from a physical therapy website? Does anyone know any good online physical therapy stores that would sell one of these?

I have always been told that the towel places the shoulder in a proper "neutral" position and is easier on the joint. I always do mine with a towel.

I have a sports physiotherapist that attends my gym with all his rehab clients. I have never seen him place a towel roll under the arm for these. IMO a neutral poition is with your arm at your side. The trick to the rotator exercise is to hold your arm in that position, not rely on bracing against a towel. These exercises are not for heavy weight. I've seen many physios come and go but the one coming in to my gym is by far the best I've seen in this country.......so far. He has a brilliant success rate with rehabs. Just my 2 cents!!
 
When I tore my labrum I had to do internal/exernal rotations and my physical therapist had me use a towel because I couldn't keep my elbow in tight when doing the exercises. The towel had me concetrate on keeping my elbow in the correct position. If the towel fell it meant my arm wasn't staying tight
 
That's the way they had me do them when I went for physical threapy after rehabbing both shoulders after surgery. They also had a little pad they used. I do them now with no extra padding - seems to work ok.
