tears / pulls / cramps

Just curious how often you guys have gotten injured with a serious pull or a tear while on. Or get cramps. In the past I would get 1 or 2 pulls or tears a year and cramps in my legs occasionally after training. I thought it was just part of the game. But lately I've been getting cramps a lot, most often in the hamstring / quads / abs. I also just tore my hamstring which had been cramping a lot. I drink as much water as I can (gallon +) I eat bananas, supplement potassium, warm up and keep warm while training. I never go excessively heavy where I know my body cannot handle the weight. Example, I tore my hamstring on a 500lb deadlift which is only about 70% of my 1RM. I also catch myself tensing up for no reason which leads to muscle spasms / cramps. Does this sound familiar to anyone or do I have a problem?
 
Pulled my calf muscle running, forgot to perform pre-stretch.

Always stretch real good, before and after workouts, and that will prevent a lot of injuries.
